Indications
- Symptomatic relief of Osteoarthritis (OA)
- Symptomatic relief of Rheumatoid arthritis (RA)
- Symptomatic relief of Ankylosing spondylitis
- Symptomatic relief of acute gouty arthritis
- Short-term treatment of moderate pain associated with dental surgery
Mode of action
- Potent, orally active cyclooxygenase-2 (COX-2) specific inhibitor
- Inhibition of COX-2 decreases clinical signs and symptoms with decreased potential for gastrointestinal toxicity
Dosage and administration
- Recommended dose for Osteoarthritis: 30 mg once daily, can be increased to 60 mg
- Recommended dose for Rheumatoid arthritis: 90 mg once daily
- Recommended dose for Ankylosing spondylitis: 90 mg once daily
- Recommended dose for Acute gouty arthritis: 120 mg once daily for 8 days
- Recommended dose for Postoperative dental surgery pain: 90 mg once daily, max 3 days
Interaction
- May interact with oral anticoagulants
- May reduce the effect of diuretics and antihypertensive drugs
- Can be used concomitantly with low-dose acetylsalicylic acid for cardiovascular prophylaxis
- Coadministration with ciclosporin or tacrolimus may increase nephrotoxic effect
- Decreases lithium renal excretion and increases plasma levels
Contraindications
- Hypersensitivity to the active substance or any excipients
- Active peptic ulceration or gastrointestinal bleeding
- History of allergic-type reactions to acetylsalicylic acid or NSAIDs
- Pregnancy, lactation, severe hepatic dysfunction
- Estimated renal creatinine clearance <30 ml/min, inflammatory bowel disease
Side effects
- Palpitation, fatigue, influenza-like symptoms, ecchymosis
- Less commonly dry mouth, taste disturbance, mouth ulcer, appetite and weight change
- Very rarely confusion and hallucinations
Pregnancy and lactation
- Not recommended in women attempting to conceive
- Not recommended for breastfeeding women
Precautions and warnings
- Caution advised in elderly and patients with prior history of gastrointestinal disease
- Careful consideration in patients with significant cardiovascular risk factors
- Monitoring of renal function should be considered
- Exercise caution in patients with cardiac failure, left ventricular dysfunction, hypertension, or liver dysfunction
- Discontinue at first appearance of skin rash or hypersensitivity
Overdose effects
- Single doses up to 500 mg and multiple doses up to 150 mg/day did not result in significant toxicity
- Usual supportive measures recommended in the event of overdose
