Indications
- Osteoarthritis

Pharmacology
- Potent COX-2 inhibitor
- Inhibition of prostaglandin synthesis
- Decreased potential for gastric toxicity
- Decreased effects on platelet aggregation
Dosage & Administration
- 30 mg once daily for osteoarthritis
- 90 mg once daily for rheumatoid arthritis and ankylosing spondylitis
- 120 mg once daily for acute gouty arthritis
- 90 mg once daily for postoperative dental surgery pain
- Shortest duration possible and lowest effective daily dose recommended
- Periodic re-evaluation of symptomatic relief and therapy response
Interaction
- Increase in prothrombin time with oral anticoagulants
- Reduction in effect of diuretics and antihypertensive drugs with NSAIDs
- Concomitant use with low-dose acetylsalicylic acid for cardiovascular prophylaxis
- Possible increased nephrotoxic effect with ciclosporin and tacrolimus
- Decreased lithium renal excretion leading to increased plasma levels
- Take without regards to meals
Contraindications
- Hypersensitivity to active substance or excipients
- Active peptic ulceration or gastrointestinal bleeding
- Bronchospasm, nasal polyps, or allergic-type reactions after taking NSAIDs
- Pregnancy, lactation, severe hepatic dysfunction, estimated renal creatinine clearance <30 ml/min
- Children and adolescents under 16 years of age
- Inflammatory bowel disease, congestive heart failure, uncontrolled hypertension, established ischemic heart disease, peripheral arterial disease, or cerebrovascular disease
Side Effects
- Palpitation, fatigue, influenza-like symptoms
- Dry mouth, taste disturbance, mouth ulcer, appetite and weight change, atrial fibrillation, transient ischemic attack
- Chest pain, flushing, cough, dyspnea, epistaxis, anxiety, impaired mental acuity
- Paraesthesia, electrolyte disturbance, myalgia, arthralgia
- Confusion and hallucinations (very rare)
Pregnancy & Lactation
- Not recommended in women attempting to conceive
- Excretion in lactating rats but not known in human milk
- Not to be used during breastfeeding
Precautions & Warnings
- Caution in patients at risk of gastrointestinal complications with NSAIDs
- Consideration in patients with significant cardiovascular event risk factors
- Monitoring of renal function, especially in patients with impaired renal blood flow
- Caution in patients with history of cardiac failure, left ventricular dysfunction, hypertension, pre-existing edema
- Monitoring and discontinuation on signs of hepatic insufficiency or abnormal liver function tests
- Discontinuation at first appearance of hypersensitivity signs
- Caution when co-administering with warfarin or other oral anticoagulants
Overdose Effects
- No significant toxicity up to 500 mg single dose or 150 mg/day for 21 days
- Usual supportive measures in case of overdose
