Indications

Pharmacology
- Potent COX-2 specific inhibitor
- Inhibition of COX-2 decreases pain, inflammation, and fever
- Selective COX-2 inhibition
- No inhibition of COX-1 at clinical doses
Dosage & Administration
- 30 mg once daily for OA (increased to 60 mg if insufficient relief)
- 90 mg once daily for RA and ankylosing spondylitis
- 120 mg once daily for acute gouty arthritis
- 90 mg once daily for postoperative dental surgery pain (up to 3 days)
- Shortest duration and lowest effective dose recommended
Interactions
- Increases prothrombin time with oral anticoagulants
- May reduce the effect of diuretics and antihypertensive drugs
- Concomitant use with low-dose acetylsalicylic acid permissible
- Possible increase in nephrotoxic effect with ciclosporin or tacrolimus
- Decreases lithium renal excretion and increases plasma levels
Contraindications
- Hypersensitivity to active substance or excipients
- Active peptic ulceration or gastrointestinal bleeding
- Bronchospasm or allergic-type reactions to NSAIDs
- Pregnancy, lactation, severe hepatic dysfunction, estimated renal clearance <30 ml/min, under 16 years of age, inflammatory bowel disease, congestive heart failure, uncontrolled hypertension, established cardiovascular diseases

Precautions & Warnings
- Caution in patients at risk for gastrointestinal complications with NSAIDs
- Consideration for patients with significant cardiovascular risk factors
- May impair renal function and blood flow in susceptible patients
- Exercise caution in patients with history of cardiac failure, edema, or liver dysfunction
- Discontinue at signs of hypersensitivity or hepatic insufficiency
- May mask fever and signs of inflammation
Overdose Effects
- Single doses up to 500 mg and multiple doses up to 150 mg/day did not result in significant toxicity
- Employ usual supportive measures in case of overdose
