Indications
- Symptomatic relief of Osteoarthritis
- Symptomatic relief of Rheumatoid arthritis
- Symptomatic relief of Ankylosing spondylitis
- Symptomatic relief of acute gouty arthritis
- Short-term treatment of moderate pain associated with dental surgery
Pharmacology
- Potent, orally active cyclooxygenase-2 (COX-2) specific inhibitor
- Inhibition of COX-2 decreases signs and symptoms of pain and inflammation
- Dose-dependent inhibition of COX-2 without inhibition of COX-1 at clinical doses up to 150 mg daily
Dosage & Administration
- Recommended dose for Osteoarthritis: 30 mg once daily, may increase to 60 mg if needed
- Recommended dose for Rheumatoid arthritis: 90 mg once daily
- Recommended dose for Ankylosing spondylitis: 90 mg once daily
- Recommended dose for acute gouty arthritis: 120 mg once daily for 8 days
- Recommended dose for postoperative dental surgery pain: 90 mg once daily for a maximum of 3 days
- Shortest duration possible and lowest effective daily dose should be used
Interaction
- Interaction with oral anticoagulants causing an increase in prothrombin time
- Impact on diuretics, ACE inhibitors, and Angiotensin II Antagonists
- Concomitant use with low-dose acetylsalicylic acid
- Potential interaction with ciclosporin, tacrolimus, and lithium
Contraindications
- Hypersensitivity to the active substance or excipients
- Active peptic ulceration or gastrointestinal bleeding
- Bronchospasm, acute rhinitis, urticaria after taking acetylsalicylic acid or NSAIDs
- Pregnancy, lactation, severe hepatic dysfunction, estimated renal creatinine clearance <30 ml/min
- Children and adolescents under 16 years
- Inflammatory bowel disease, congestive heart failure, hypertension, established cardiovascular diseases
Side Effects
- Palpitation, fatigue, influenza-like symptoms
- Dry mouth, taste disturbance, mouth ulcer
- Atrial fibrillation, chest pain, flushing
- Anxiety, impaired mental acuity, electrolyte disturbance
- Myalgia, arthralgia, confusion, hallucinations
Pregnancy & Lactation
- Not recommended in women attempting to conceive
- Excretion in lactating rats, women must not breastfeed while using the medication
Precautions & Warnings
- Caution with patients at risk of gastrointestinal complications with NSAIDs
- Consideration for patients with significant cardiovascular risk factors
- Monitoring of renal function, exercise caution in patients with cardiac failure or hypertension
- Monitoring for liver function and signs of hepatic insufficiency
- Discontinue at the first appearance of hypersensitivity signs, and caution with co-administration with oral anticoagulants
Overdose Effects
- Administration of single/multiple doses within specified limits did not result in significant toxicity
- Usual supportive measures in case of overdose
