Indications
- Symptomatic relief of Osteoarthritis (OA)
- Symptomatic relief of Rheumatoid arthritis (RA)
- Symptomatic relief of Ankylosing spondylitis
- Symptomatic relief of acute gouty arthritis
- Short-term treatment of moderate pain associated with dental surgery
Pharmacology
- Potent, orally active cyclooxygenase-2 (COX-2) specific inhibitor
- Inhibition of COX-2 decreases clinical signs and symptoms
- Selective inhibition of COX-2 with decreased potential for GI toxicity and effects on platelet aggregation
Dosage & Administration
- Recommended dose for Osteoarthritis: 30 mg once daily, may increase to 60 mg once daily
- Recommended dose for Rheumatoid arthritis: 90 mg once daily
- Recommended dose for Ankylosing spondylitis: 90 mg once daily
- Recommended dose for Acute gouty arthritis: 120 mg once daily
- Recommended dose for Postoperative dental surgery pain: 90 mg once daily, limited to a maximum of 3 days
Interaction
- Interaction with oral anticoagulants leading to increase in prothrombin time
- Potential reduction in the effect of diuretics and other antihypertensive drugs when taken concomitantly
- Can be used concomitantly with low-dose acetylsalicylic acid for cardiovascular prophylaxis
- Potential increase in nephrotoxic effect of ciclosporin or tacrolimus when coadministered with NSAIDs
- NSAIDs decrease lithium renal excretion and increase lithium plasma levels
Contraindications
- Hypersensitivity to the active substance or excipients
- Active peptic ulceration or active gastrointestinal (GI) bleeding
- Patients who have experienced bronchospasm or allergic-type reactions after taking acetylsalicylic acid or NSAIDs
- Pregnancy and lactation
- Severe hepatic dysfunction, estimated renal creatinine clearance <30 ml/min, children and adolescents under 16 years of age, inflammatory bowel disease, congestive heart failure, uncontrolled hypertension, established ischaemic heart disease, peripheral arterial disease, and/or cerebrovascular disease

Precautions & Warnings
- Caution with patients at risk of developing gastrointestinal complications
- Caution with patients having significant risk factors for cardiovascular events
- May cause reduction in prostaglandin formation and impair renal function
- Caution in patients with a history of cardiac failure, left ventricular dysfunction, or hypertension
- Monitoring of renal function and liver function tests recommended
- Discontinue at the first appearance of hypersensitivity signs
- May mask fever and other signs of inflammation
- Caution when co-administering with warfarin or other oral anticoagulants
Overdose Effects
- Single doses up to 500 mg and multiple doses up to 150 mg/day did not result in significant toxicity
- Usual supportive measures may be employed in case of overdose
