Indications
- Symptomatic relief of osteoarthritis (OA)
- Symptomatic relief of rheumatoid arthritis (RA)
- Symptomatic relief of ankylosing spondylitis
- Symptomatic relief of acute gouty arthritis pain and signs of inflammation
- Short-term treatment of moderate pain associated with dental surgery
Pharmacology
- Potent, orally active COX-2 specific inhibitor
- Selectively inhibits COX-2 without affecting COX-1 at clinical dose range
- Decreases clinical signs and symptoms with decreased potential for GI toxicity
- Decreases effects on platelet aggregation
- Prostaglandin-mediated normal physiologic functions
- Decreases prostaglandin synthesis
Dosage & Administration
- Recommended doses for osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, acute gouty arthritis, and postoperative dental surgery pain
- Possible need for increased dose in some patients
- Shortest duration and lowest effective daily dose usage as possible
- Re-evaluation of patient's need for symptomatic relief and therapy response
Interaction
- Interaction with oral anticoagulants
- Interaction with diuretics, ACE inhibitors, and angiotensin II antagonists
- Concomitant use with acetylsalicylic acid at doses used for cardiovascular prophylaxis
- Potential interaction with ciclosporin and tacrolimus
- NSAID-induced decrease in lithium renal excretion
- Advice to take without regards to meals
Contraindications
- Hypersensitivity to the active substance or excipients
- Active peptic ulceration or active GI bleeding
- Bronchospasm, acute rhinitis, nasal polyps, and allergic-type reactions after taking NSAIDs
- Pregnancy and lactation
- Severe hepatic dysfunction
- Estimated renal creatinine clearance below 30 ml/min
- Children and adolescents under 16 years of age
- Inflammatory bowel disease
- Congestive heart failure, established ischaemic heart disease, peripheral arterial disease, and/or cerebrovascular disease

Precautions & Warnings
- Caution in patients at risk of developing GI complications with NSAIDs
- Caution in patients with significant risk factors for cardiovascular events
- Potential impact on renal function and renal blood flow
- Caution in patients with history of cardiac failure, left ventricular dysfunction, or hypertension
- Monitoring of liver function and discontinuation upon signs of hepatic insufficiency
- Discontinuation upon first appearance of skin rash, mucosal lesions, or hypersensitivity signs
- Potential masking of fever and other signs of inflammation
- Exercise caution when co-administering with warfarin or other oral anticoagulants
Overdose Effects
- No significant toxicity observed up to single doses of 500 mg and multiple doses of 150 mg/day for 21 days
- Employment of usual supportive measures in case of overdose
