Indications
- Symptomatic relief of Osteoarthritis (OA)
- Symptomatic relief of Rheumatoid arthritis (RA)
- Symptomatic relief of Ankylosing spondylitis
- Symptomatic relief of acute gouty arthritis
- Short-term treatment of moderate pain associated with dental surgery
Pharmacology
- Potent, orally active cyclooxygenase-2 (COX-2) specific inhibitor
- Decreases clinical signs and symptoms with decreased potential for GI toxicity and effects on platelet aggregation
- Did not inhibit gastric prostaglandin synthesis
Dosage & Administration
- Dosage for Osteoarthritis: 30 mg once daily, increased to 60 mg once daily if insufficient relief
- Dosage for Rheumatoid arthritis: 90 mg once daily
- Dosage for Ankylosing spondylitis: 90 mg once daily
- Dosage for Acute gouty arthritis: 120 mg once daily for 8 days
- Dosage for Postoperative dental surgery pain: 90 mg once daily, limited to a maximum of 3 days
Interaction
- Increased prothrombin time with oral anticoagulants
- NSAIDs may reduce the effect of diuretics and other antihypertensive drugs
- Can be used concomitantly with low-dose acetylsalicylic acid for cardiovascular prophylaxis
Contraindications
- Hypersensitivity to active substance or excipients
- Active peptic ulceration or active GI bleeding
- Patients who have experienced allergic-type reactions to NSAIDs
- Pregnancy
- Severe hepatic dysfunction
- Estimated renal creatinine clearance < 30 ml/min
- Inflammatory bowel disease
- Congestive heart failure
- Patients with hypertension not adequately controlled

Pregnancy & Lactation
- Not recommended in women attempting to conceive
- Not known whether excreted in human milk
- Must not breastfeed
Precautions & Warnings
- Caution advised with treatment of patients at risk of developing GI complication with NSAIDs
- Treatment of patients with significant risk factors for cardiovascular events only after careful consideration
- Monitoring of renal function in patients at risk
- Caution in patients with history of cardiac failure, left ventricular dysfunction, hypertension, and pre-existing edema
- Monitoring of hepatic function tests, discontinuation at signs of hepatic insufficiency or hypersensitivity
- May mask fever and other signs of inflammation, caution with co-administration with warfarin or other oral anticoagulants
Overdose Effects
- Administration of single doses up to 500 mg and multiple doses up to 150 mg/day for 21 days did not result in significant toxicity
- Usual supportive measures recommended in the event of overdose
