Indications
- Relief of osteoarthritis pain
- Rheumatoid arthritis symptom relief
- Ankylosing spondylitis relief
- Acute gouty arthritis relief
- Moderate pain relief post dental surgery
Pharmacology
- Cyclooxygenase-2 (COX-2) specific inhibitor
- Inhibition of prostanoid mediators of pain, inflammation, and fever
Dosage & Administration
- 30mg once daily for OA
- 90mg once daily for RA and AS
- 120mg once daily for acute gouty arthritis
- 90mg once daily for postoperative dental surgery pain
- Shortest duration possible and lowest effective daily dose should be used
- Re-evaluate patient's need for symptomatic relief and response to therapy periodically
Interaction
- Increase in prothrombin time with oral anticoagulants
- Reduction in effect of diuretics and antihypertensive drugs with NSAIDs
- Concomitant use with low-dose acetylsalicylic acid for cardiovascular prophylaxis is possible
- Potential to increase nephrotoxic effect of ciclosporin or tacrolimus
- Decrease in lithium renal excretion leading to increased plasma levels
- Take without regards to meals
Contraindications
- Hypersensitivity to active substance or excipients
- Active peptic ulceration or gastrointestinal bleeding
- Bronchospasm or allergic-type reactions to NSAIDs or COX-2 inhibitors
- Pregnancy, lactation, severe hepatic dysfunction, estimated renal creatinine clearance <30 ml/min, children and adolescents under 16 years of age, inflammatory bowel disease, congestive heart failure, uncontrolled hypertension, established ischemic heart disease/ peripheral arterial disease/ cerebrovascular disease

Pregnancy & Lactation
- Not recommended in women attempting to conceive
- Excreted in the milk of lactating rats
- Not safe for breastfeeding women
Precautions & Warnings
- Caution with NSAID treatment in elderly, concomitant use with other NSAIDs or acetylsalicylic acid, prior history of gastrointestinal disease
- Caution in patients with significant risk factors for cardiovascular events
- Monitoring of renal function in patients with reduced prostaglandin formation and impaired renal function
- Exercise caution in patients with cardiac failure, left ventricular dysfunction, hypertension, or edema
- Monitor patients with symptoms or signs suggesting liver dysfunction, discontinue at first sign of hypersensitivity
- May mask fever and other signs of inflammation, use with caution with oral anticoagulants
Overdose Effects
- Single doses up to 500 mg and multiple doses up to 150 mg/day did not result in significant toxicity
- Usual supportive measures recommended in case of overdose
