Indications
- Symptomatic relief of Osteoarthritis (OA)
- Symptomatic relief of Rheumatoid arthritis (RA)
- Symptomatic relief of Ankylosing spondylitis
- Symptomatic relief of pain and signs of inflammation associated with acute gouty arthritis
- Short-term treatment of moderate pain associated with dental surgery
Pharmacology
- Potent, orally active cyclooxygenase-2 (COX-2) specific inhibitor
- Inhibition of COX-2 decreases clinical signs and symptoms
- Selective inhibition of COX-2 decreases potential for GI toxicity and effects on platelet aggregation
- Did not inhibit gastric prostaglandin synthesis
- Decreases potential for GI toxicity and effects on platelet aggregation
Dosage & Administration
- Recommended adult dose for Osteoarthritis: 30 mg once daily (60 mg once daily in some cases)
- Recommended adult dose for Rheumatoid arthritis: 90 mg once daily
- Recommended adult dose for Ankylosing spondylitis: 90 mg once daily
- Recommended adult dose for Acute gouty arthritis: 120 mg once daily for 8 days
- Recommended adult dose for Postoperative dental surgery pain: 90 mg once daily for maximum of 3 days
- Shortest duration possible and lowest effective daily dose should be used
Interaction
- Increase in prothrombin time when administered with oral anticoagulants
- May reduce the effect of diuretics and other antihypertensive drugs when administered with diuretics, ACE inhibitors, and Angiotensin II Antagonists
- Can be used concomitantly with low-dose acetylsalicylic acid for cardiovascular prophylaxis
- Coadministration with ciclosporin or tacrolimus may increase nephrotoxic effect
- Decreases lithium renal excretion and increases lithium plasma levels when administered with lithium
Contraindications
- Hypersensitivity to active substance or excipients
- Active peptic ulceration or active gastrointestinal (GI) bleeding
- Patients with bronchospasm, acute rhinitis, nasal polyps, angioneurotic oedema, urticaria, or allergic-type reactions after taking acetylsalicylic acid or NSAIDs
- Pregnancy and lactation
- Severe hepatic dysfunction
- Estimated renal creatinine clearance <30 ml/min
- Children and adolescents under 16 years of age
- Inflammatory bowel disease
- Congestive heart failure (NYHA ll-IV)
- Uncontrolled hypertension, established ischaemic heart disease, peripheral arterial disease, and/or cerebrovascular disease

Pregnancy & Lactation
- Not recommended during attempted conception
- Not known whether excreted in human milk
- Not recommended for breastfeeding women
Precautions & Warnings
- Caution advised for patients at risk of developing gastrointestinal complication with NSAIDs
- Caution in patients with significant risk factors for cardiovascular events
- Monitoring of renal function in patients with reduced prostaglandin formation and impaired renal function
- Caution in patients with history of cardiac failure, left ventricular dysfunction, or hypertension
- Monitoring advised for signs of hepatic insufficiency
- Discontinue at first appearance of hypersensitivity reactions
- May mask fever and other signs of inflammation, caution with co-administration with oral anticoagulants
Overdose Effects
- Single doses up to 500 mg and multiple doses up to 150 mg/day did not result in significant toxicity
- Usual supportive measures recommended in the event of overdose
