Indications
- Symptomatic relief of Osteoarthritis (OA)
- Symptomatic relief of Rheumatoid arthritis (RA)
- Symptomatic relief of Ankylosing spondylitis
- Symptomatic relief of acute gouty arthritis
- Short-term treatment of moderate pain associated with dental surgery
Pharmacology
- Potent, orally active COX-2 specific inhibitor
- Inhibition of COX-2 decreases clinical signs and symptoms
- Decreased potential for Gl toxicity and effects on platelet aggregation
- Dose-dependent inhibition of COX-2 without inhibition of COX-1 at doses up to 150 mg daily
- No inhibition of gastric prostaglandin synthesis
Dosage & Administration
- Recommended doses for Osteoarthritis, Rheumatoid arthritis, Ankylosing spondylitis, acute gouty arthritis, and postoperative dental surgery pain
- Shortest duration possible and lowest effective daily dose should be used
- Patients may require additional postoperative analgesia
- Need for symptomatic relief and therapy response should be re-evaluated periodically
Interaction
- Interaction with oral anticoagulants
- Potential interaction with diuretics, ACE inhibitors, Angiotensin II Antagonists, acetylsalicylic acid, ciclosporin, tacrolimus, and lithium
- Take without regards to meals
Contraindications
- Hypersensitivity to the active substance or its excipients
- Active peptic ulceration or active Gl bleeding
- Bronchospasm, acute rhinitis, nasal polyps, allergic-type reactions after taking acetylsalicylic acid or NSAIDs
- Pregnancy, lactation, severe hepatic dysfunction, estimated renal creatinine clearance
- Children and adolescents under 16 years of age, inflammatory bowel disease, congestive heart failure, uncontrolled hypertension, established cardiovascular diseases
Side Effects
- Palpitation, fatigue, influenza-like symptoms, ecchymosis
- Less commonly dry mouth, taste disturbance, mouth ulcer, appetite and weight change
- Very rarely confusion and hallucinations
Pregnancy & Lactation
- Not recommended for women attempting to conceive
- Not recommended for breastfeeding women
- Excreted in the milk of lactating rats
Precautions & Warnings
- Caution with patients at risk of gastrointestinal complications with NSAIDs
- Caution with patients at risk of cardiovascular events
- Monitoring of renal function and caution in patients with history of cardiac failure, left ventricular dysfunction, hypertension, and pre-existing edema
- Monitoring of hepatic function and discontinuation if signs of hepatic insufficiency occur
- Discontinuation at the first appearance of hypersensitivity signs
- Caution when co-administering with warfarin or other oral anticoagulants
Overdose Effects
- Single doses up to 500 mg and multiple doses up to 150 mg/day did not result in significant toxicity
- Usual supportive measures recommended in the event of overdose
