Pharmacological action
- Potent orally active cyclooxygenase-2 (COX-2) specific inhibitor
Indications
- Relief of Osteoarthritis (OA)
- Relief of Rheumatoid arthritis (RA)
- Relief of Ankylosing spondylitis
- Relief of pain and signs of inflammation associated with acute gouty arthritis
- Short-term treatment of moderate pain associated with dental surgery
Dosage and administration
- Recommended dose for Osteoarthritis: 30 mg once daily
- Recommended dose for Rheumatoid arthritis: 90 mg once daily
- Recommended dose for Ankylosing spondylitis: 90 mg once daily
- Recommended dose for Acute gouty arthritis: 120 mg once daily for 8 days
- Recommended dose for Postoperative dental surgery pain: 90 mg once daily for maximum 3 days
Contraindications
- Hypersensitivity to the active substance or excipients
- Active peptic ulceration or active gastro-intestinal (GI) bleeding
- Patients with bronchospasm, nasal polyps or allergic reactions to acetylsalicylic acid or NSAIDs
- Pregnancy and lactation
- Severe hepatic dysfunction
- Estimated renal creatinine clearance <30 ml/min
- Children and adolescents under 16 years of age
- Inflammatory bowel disease
- Congestive heart failure
- Patients with hypertension not adequately controlled
- Established ischaemic heart disease, peripheral arterial disease, and/or cerebrovascular disease

Precautions warnings
- Caution advised for patients at risk of developing gastrointestinal complications with NSAIDs
- Caution for patients with significant cardiovascular risk factors
- Monitoring of renal function recommended
- Caution in patients with a history of cardiac failure, left ventricular dysfunction, or hypertension
- Monitoring of liver function recommended
- Discontinue at first appearance of hypersensitivity signs
- May mask fever and other signs of inflammation
- Exercise caution when co-administering with warfarin or other oral anticoagulants
Overdose effects
- No significant toxicity observed at single doses up to 500 mg
- Supportive measures recommended in case of overdose
Interaction with medications
- Increase in prothrombin time with chronic warfarin therapy
- Potential reduction in the effect of diuretics and other antihypertensive drugs
- Can be used concomitantly with acetylsalicylic acid at doses used for cardiovascular prophylaxis
- Potential nephrotoxic effect increase with ciclosporin or tacrolimus coadministration
- NSAIDs can decrease lithium renal excretion and increase lithium plasma levels
