Description
- Atonor is indicated to reduce elevated total cholesterol, LDL cholesterol, apolipoprotein B (Apo-B) and triglycerides levels in various diseases
- Selectively inhibits HMG-CoA reductase enzyme responsible for cholesterol synthesis in the liver
- Rapidly absorbed after oral administration with maximum plasma concentrations occurring within 1 to 2 hours
- Metabolized to ortho- and parahydroxylated derivatives and various beta-oxidation products
- Eliminated primarily in bile following hepatic and/or extra-hepatic metabolism
Dosage & Administration
- Adult dose: Usually 10 mg once daily; may be increased to a maximum of 80 mg once daily if necessary
- Child (10-18 years) dose: Initially 10 mg once daily, increased if necessary to a usual max of 20 mg once daily
- Dosage range of 10 to 80 mg once daily; administered with or without food
- Plasma elimination half-life approximately 14 hours, with active metabolites contributing to HMG-CoA reductase inhibitory activity
Interactions
- Increased risk of myopathy with concurrent use of cyclosporine, fibric acid derivatives, niacin, and azole antifungals
- Altered plasma concentrations when coadministered with antacids, colestipol, digoxin, erythromycin, oral contraceptives, and warfarin
Contraindications
- Hypersensitivity to any component of the medication
- Active liver disease or unexplained persistent elevations of serum transaminases
- History of serious adverse reaction to prior administration of HMG-CoA reductase inhibitors
Side Effects
- Constipation, flatulence, dyspepsia, abdominal pain
- Infection, headache, back pain, rash, asthenia, arthralgia, myalgia
Pregnancy & Lactation
- Contradicted during pregnancy and breastfeeding due to potential fetal and infant adverse reactions
Precautions & Warnings
- Liver function tests should be performed before initiation of treatment and periodically thereafter
- Discontinue therapy if markedly elevated CPK levels occur or myopathy is diagnosed or suspected
Use in Special Populations
- Higher plasma concentrations in healthy elderly subjects compared to young adults
- No influence on plasma concentrations or LDL-C reduction in patients with renal insufficiency
Overdose Effects
- No specific treatment available; symptomatic and supportive measures should be instituted
- Liver function tests and serum CK levels should be monitored
