Indications
- Relief of rheumatoid arthritis
- Treatment of degenerative joint diseases
- Alleviation of musculoskeletal and joint disorders such as tendinitis and sprain
- Managing pain from dental issues, headache, and primary dysmenorrhea
Pharmacology
- NSAID with anti-inflammatory, analgesic, and antipyretic actions
- Inhibition of prostaglandin synthesis
- Stabilization of lysosomal membranes
- Inhibition of leukotriene synthesis
- Antibradykinin activity
- Rapid and almost complete absorption from the GI tract
- Approximately 99% bound to plasma protein, mainly albumin
- Rapid and extensive liver metabolism via conjugation with glucoronic acid
- Excretion through urine and feces
- Peak concentration reached at about 0.5-0.75 hour after IM injection
- Elimination half-life of approximately 1.88 hour
Dosage
- Recommended starting dose of 150 to 300 mg/day in 3 divided doses for anti-inflammatory purposes
- Usual recommended dose of 25 to 50 mg, every 6 to 8 hours for pain management
- Maximum daily dose of 300 mg
Administration
- Oral forms to be taken with fluids, preferably with food
Interaction
- Not recommended drug associations with other NSAIDs and high dose salicylates
- Increased risk of bleeding with anticoagulants
- Increased risk of elevation of lithium plasma levels with lithium
- Increased risk of haematologic toxicity of methotrexate at doses greater than 15mg/week
- Risk of hyperkalemia with certain medicinal products and therapeutic categories
- Increased risk of gastrointestinal ulceration or bleeding with corticosteroids
- Patients taking diuretics at greater risk of developing renal failure
- Possible deterioration of renal function with compromised renal function and co-administration of ACE inhibitors or Angiotensin II Antagonists
- Increased risk for severe complications with concomitant administration of nicorandil and NSAIDs
- Potential pharmacokinetic interaction between ketoprofen and digoxin
- Increased risk of nephrotoxicity with cyclosporin and tacrolimus
- Monitoring of full blood count during combination treatment with methotrexate at doses lower than 15mg/week
- More frequent clinical monitoring and monitoring of bleeding time required with pentoxifylline
- Increased risk of renal failure with concomitant administration of tenofovir disoproxil fumarate and NSAIDs
- Risk of decreased antihypertensive potency with antihypertensive agents
- Increased risk of bleeding with thrombolytics and increased risk of gastrointestinal bleeding with selective serotonin reuptake inhibitors (SSRIs)
Contraindications
- History of hypersensitivity reactions to ketoprofen, ASA or other NSAIDs
- Severe heart failure
- Active or history of peptic ulcer/hemorrhage
- History of gastrointestinal bleeding or perforation related to previous NSAIDs therapy
- Severe hepatic insufficiency
- Severe renal insufficiency
- Third trimester of pregnancy
- Rectitis or history of proctorrhagia (rectal administration)
Side Effects
- Rare: haemorrhagic anaemia
- Unknown: agranulocytosis, thrombocytopenia, bone marrow failure, hemolytic anemia, leucopenia
- Unknown: anaphylactic reactions (including shock)
- Unknown: depression, hallucinations, confusion, altered mood
- Uncommon: headache, dizziness, somnolence
- Rare: paraesthesia
- Unknown: aseptic meningitis, convulsions, dysgeusia, vertigo
- Rare: vision blurred
- Rare: tinnitus
- Unknown: exacerbation of heart failure, atrial fibrillation
- Unknown: hypertension, vasodilatation, vasculitis

- Unknown: bronchospasm (particularly in patients with known hypersensitivity to ASA and other NSAIDs)
- Common: dyspepsia, nausea, abdominal pain, vomiting
- Uncommon: constipation, diarrhea, flatulence, gastritis

- Unknown: exacerbation of colitis and Crohn’s disease, gastrointestinal haemorrhage and perforation, pancreatitis

Pregnancy & Lactation
- Avoid use during the first and second trimester of pregnancy
- Contraindicated during the last trimester of pregnancy
- Not recommended in nursing mothers
Precautions & Warnings
- Careful administration to patients with a history of gastrointestinal disease
- Renal function monitoring for patients with heart failure, cirrhosis, nephrosis, diuretic therapy, or chronic renal impairment
- Caution required in patients with a history of hypertension and/or congestive heart failure
- Monitoring for atrial fibrillation
- Potential occurrence of hyperkalemia
- Masking of signs of infection progression such as fever
- Periodic evaluation of liver function tests in patients with abnormal liver function or a history of liver disease
- Discontinuation if visual disturbances occur
- Impairment of female fertility
- Consideration of withdrawal in women attempting to conceive or undergoing infertility investigation
Use in Special Populations
