Description
- Symptomatic treatment of rheumatoid arthritis
- Management of degenerative joint diseases
- Relief of musculoskeletal and joint disorders such as tendinitis and sprain
- Treatment of pain, including dental pain, headache, and primary dysmenorrhea
- Non-steroidal anti-inflammatory drug (NSAID) with analgesic and antipyretic actions
- Rapid and complete absorption from the GI tract
- Approximately 99% bound to plasma protein, mainly albumin
- Excretion in urine (50-90%) and faeces (1-8%) within 1-5 days
- Peak concentration of approximately 10 mg/L reached at about 0.5-0.75 hour after 100 mg IM injection
- Elimination half-life of approximately 1.88 hours
- Oral forms should be taken with fluids, preferably with food
- Contraindicated in patients with hypersensitivity reactions, severe heart failure, peptic ulcer/hemorrhage, severe hepatic or renal insufficiency, and in the third trimester of pregnancy
- Not recommended during the first and second trimesters of pregnancy and in nursing mothers
- Caution required in patients with a history of gastrointestinal disease, renal impairment, hypertension, congestive heart failure, and infectious disease
- Discontinue treatment if visual disturbances occur
- Reduced initial dosage and careful monitoring in elderly patients, those with hepatic or renal impairment
- Store below 30°C, protect from light, and do not use after the expiry date
Dosage
- Starting dose: 150-300 mg/day in 3 divided doses for anti-inflammatory purposes
- Recommended maintenance dosage: 100-200 mg/day
- Usual dose for pain and primary dysmenorrhea: 25-50 mg every 6-8 hours, not to exceed 300 mg/day
Drug Interaction
- Increased risk of gastrointestinal ulceration and bleeding with other NSAIDs and high-dose salicylates
- Increased risk of bleeding with anticoagulants like heparin, warfarin, and platelet aggregation inhibitors
- Coadministration with lithium may elevate lithium plasma levels
- Risk of haematologic toxicity of methotrexate, particularly at high doses
- Potential for drug interactions leading to hyperkalemia
Side Effects
- Rare: Haemorrhagic anemia, blurred vision, tinnitus
- Uncommon: Headache, dizziness, somnolence, dyspepsia, nausea, abdominal pain
- Unknown: Agranulocytosis, thrombocytopenia, depression, hallucinations, hypertension, exacerbation of heart failure, asthma, hepatitis, rash
- Potential for renal failure and impaired female fertility
Pregnancy and Lactation
- Avoid use during the first and second trimesters of pregnancy
- Contraindicated in the last trimester of pregnancy
- Not recommended in nursing mothers
Precautions and Warnings
- Use with caution in patients with a history of gastrointestinal disease, heart failure, and chronic renal impairment
- Monitor renal function in patients at risk of fluid retention and oedema
- Caution required in patients with abnormal liver function tests or visual disturbances
- Avoid use in women attempting to conceive
- Minimize dosage in elderly, hepatically or renally impaired patients
- Monitor for symptoms of overdose, especially in cases of massive overdosages
Special Populations
- Elderly: Reduce initial dosage and maintain on minimal effective dose
- Hepatic impairment: Monitor carefully and maintain at minimal effective daily dosage
- Renal impairment: Reduce initial dosage and maintain on minimal effective dose
Overdose Effects
- Symptoms include lethargy, drowsiness, nausea, vomiting, and epigastric pain
- No specific antidotes to ketoprofen overdosages
- Institute symptomatic and supportive treatment, and consider hemodialysis in cases of renal failure
- Gastric lavage recommended for massive overdosages
