Pharmacology
- Selective inhibitor of HMG-CoA reductase
- Lowers plasma cholesterol and lipoprotein levels
- Inhibits HMG-CoA reductase and cholesterol synthesis in the liver
- Increases the number of hepatic LDL receptors for enhanced uptake and catabolism of LDL
Indications
- Reduces elevated total cholesterol, LDL cholesterol, apolipoprotein B (Apo-B) and triglycerides levels
- Reduces total cholesterol and LDL cholesterol in patients with heterozygous and homozygous familial hypercholesterolaemia
- Reduces elevated cholesterol and triglycerides in patient with mixed dyslipidemia
- Treats patients with elevated serum triglyceride levels in hypertriglyceridaemia
- Treats patients with dysbetalipoproteinaemia
- Reduces cardiac ischaemic events in patients with coronary artery disease with elevated LDL-cholesterol level
- Reduces total and LDL-cholesterol concentrations in patients with hypercholesterolemia associated with diabetes mellitus or renal transplantation
Contraindications
- Hypersensitivity to any component of this medication
- Active liver disease or unexplained persistent elevations of serum transaminases
- History of serious adverse reaction to prior administration of HMG-CoA reductase inhibitors
Dosage administration
- Adults: Usually 10 mg once daily; may be increased to max. 80 mg once daily if necessary
- Children (10-18 years): Initially 10 mg once daily, increased if necessary to usual max. 20 mg once daily
Interaction
- Increased risk of myopathy with concurrent administration of cyclosporine, fibric acid derivatives, niacin, erythromycin, azole antifungals
- Antacid coadministration decreases plasma concentrations of atorvastatin
- Colestipol coadministration decreases plasma concentrations but increases LDL-C reduction
- Erythromycin coadministration increases plasma concentrations of atorvastatin
- Oral contraceptives coadministration increases AUC values for norethindrone and ethinyl estradiol
- Warfarin coadministration has no clinically significant effect on prothrombin time
Side effects
- Most frequent side effects: constipation, flatulence, dyspepsia, abdominal pain. Other side effects: infection, headache, back pain, rash, asthenia, arthralgia, myalgia
Pregnancy lactation
- Atorvastatin is contraindicated during pregnancy
- Not known whether atorvastatin or its metabolites are excreted in human milk
- Maternal treatment may reduce fetal levels of mevalonate
- Atorvastatin should not be used in women who are pregnant, trying to become pregnant or breastfeeding
Precautions warnings
- Liver function tests should be performed before initiation of treatment and periodically thereafter
- Atovin should be used with caution in patients who consume substantial quantities of alcohol or have a history of liver disease
- Therapy should be discontinued if markedly elevated CPK levels occur or myopathy is diagnosed or suspected
Special populations
- Plasma concentrations of Atovin are higher in healthy elderly subjects than in young adults
- No clinically significant difference in LDL-C reduction with Atovin between men and women
- Renal disease has no influence on the plasma concentrations or LDL-C reduction of Atovin
- Hemodialysis is not expected to significantly enhance clearance of Atovin
- Plasma concentrations of Atovin are markedly increased in patients with chronic alcoholic liver disease
Overdose effects
- Specific treatment is not available for atorvastatin overdose
- Patient should be treated symptomatically and supportive measures instituted, as required
- Liver function tests should be performed and serum CK levels should be monitored
- Hemodialysis is not expected to significantly enhance clearance of atorvastatin
