Description
- Atorvastatin is a selective inhibitor of HMG-CoA reductase.
- It is used to reduce elevated total cholesterol, LDL cholesterol, apolipoprotein B, and triglycerides levels.
- It is indicated for various diseases including familial hypercholesterolemia, mixed dyslipidemia, and hypertriglyceridemia.
- It is also used for the treatment of patients with dysbetalipoproteinaemia and to reduce cardiac ischaemic events.
- Atorvastatin is rapidly absorbed after oral administration, with maximum plasma concentrations occurring within 1 to 2 hours.
Dosage
- Adults: Usually 10 mg once daily; may be increased to max. 80 mg once daily.
- Children (10-18 years): Usual max. 20 mg once daily.
- For familial hypercholesterolaemia: Initially 10 mg daily, increased at intervals to max. 80 mg once daily.
Absorption
- Atorvastatin is rapidly absorbed after oral administration; maximum plasma concentrations occur within 1 to 2 hours.
Distribution
- Mean volume of distribution of Atorvastatin is approximately 381 liters.
Metabolism
- Atorvastatin is extensively metabolized to ortho- and parahydroxylated derivatives and various beta-oxidation products.
Excretion
- Atorvastatin and its metabolites are eliminated primarily in bile following hepatic and/or extra-hepatic metabolism.
Contraindications
- Hypersensitivity to any component of this medication
- Active liver disease or unexplained persistent elevations of serum transaminases
- History of serious adverse reaction to prior administration of HMG-CoA reductase inhibitors

Pharmacology
- Atorvastatin lowers plasma cholesterol and lipoprotein levels by inhibiting HMG-CoA reductase and cholesterol synthesis in the liver.
Interaction
- Increased risk of myopathy with concurrent administration of cyclosporine, fibric acid derivatives, niacin, erythromycin, and azole antifungals
- Coadministration with antacid suspension or colestipol may alter plasma concentrations of atorvastatin.
Pregnancy & Lactation
- Contraindicated during pregnancy
- Not known whether atorvastatin or its metabolites are excreted in human milk
Precautions & Warnings
- Liver function tests should be performed before the initiation of treatment and periodically thereafter
- Use with caution in patients who consume substantial quantities of alcohol or have a history of liver disease

Use in Special Populations
- Plasma concentrations of Atrovast are higher in healthy elderly subjects than in young adults
- No clinically significant difference in LDL-C reduction with Atrovast between men and women
- Renal disease has no influence on the plasma concentrations or LDL-C reduction of Atrovast
