Pharmacology
- Blocks COX-1 and COX-2 enzymes
- Decreases prostaglandin synthesis
- COX-1 enzyme linked to gastrointestinal upset and renal toxicities
- COX-2 enzyme mediates antipyretic, analgesic, and anti-inflammatory properties
Dosage & Administration
- Naproxen Enteric Coated Tablet for adults: 500-1000 mg per day in divided doses
- Naproxen Enteric Coated Tablet for acute gout: 750 mg initially, then 250 mg every 8 hours
- Naproxen Enteric Coated Tablet for dysmenorrhea: 500 mg initially, then 250 mg at 6-8 hour intervals for up to 5 days
- Naproxen Suspension for juvenile rheumatoid arthritis: 10 mg/kg/day in two divided doses
- Naproxen Gel: 2-6 applications per day as required
Interaction
- Diminishes antihypertensive effect of ACE inhibitors
- Delays absorption of naproxen by antacids & sucralfate
- Increases adverse effects with Aspirin
- Reduces natriuretic effect of diuretics
- Enhances toxicity of Methotrexate
- Increases risk of GI bleeding with Warfarin and Selective Serotonin Reuptake Inhibitors (SSRI)
Contraindications
- Known hypersensitivity to Naproxen
- Asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- Perioperative pain in the setting of coronary artery bypass graft (CABG) surgery
Side Effects
- Gastrointestinal: Heartburn, abdominal pain, nausea, diarrhea, dyspepsia
- Central Nervous System: Headache, vertigo, drowsiness
- Dermatological: Pruritus (itching), purpura
- Cardiovascular: Edema, palpitation
- Others: Visual disturbances, hearing disturbances
Pregnancy & Lactation
- May have adverse effects on the fetus
- Associated with premature closure of the ductus arteriosus and pulmonary hypertension in infants
- Excreted in small amounts in breast milk
- Requires careful balancing of benefits to the mother against possible risks to the fetus
- Some authorities consider the amount in breast milk too small to be harmful
Precautions & Warnings
- Long-term administration may result in abnormal renal pathology in animals
- Reports of acute interstitial nephritis and abnormal renal function in humans
- Eliminated primarily by the kidneys
- Caution with significantly impaired renal function
- Monitoring of serum creatinine and/or creatinine clearance advised
- Reports of moderate to severe jaundice attributed to Naproxen
