Indications

Pharmacology
- Blocks COX-1 and COX-2 enzymes
- Decreases prostaglandin synthesis
- COX-1 enzyme is constitutively active in normal tissues
- COX-2 enzyme produces prostaglandins for pain, fever, and inflammation
Dosage & Administration
- Oral preparation to be taken with water, preferably after meals
- Adult dosage for rheumatoid arthritis/osteoarthritis/ankylosing spondylitis
- Adult dosage for acute gout
- Adult dosage for dysmenorrhoea
- Adult dosage for other indications like analgesia and acute musculoskeletal disorders
- Children's dosage for juvenile rheumatoid arthritis

Interaction
- Diminishes antihypertensive effect of ACE inhibitors
- Delays absorption by antacids & Sucralfate
- Increases adverse effects with Aspirin
- Reduces natriuretic effect of Diuretics
- Enhances toxicity of Methotrexate
- Increases risk of GI bleeding with Warfarin and Selective Serotonin Reuptake Inhibitors (SSRI)
Contraindications
- Known hypersensitivity to Naproxen
- Asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- Treatment of perioperative pain in the setting of coronary artery bypass graft (CABG) surgery
Side Effects
- Gastrointestinal: Heartburn, abdominal pain, nausea, diarrhea, dyspepsia
- Central Nervous System: Headache, vertigo, drowsiness
- Dermatological: Pruritus (itching), purpura
- Cardiovascular: Edema, palpitation
- Others: Visual disturbances, hearing disturbances
Pregnancy & Lactation
- May have adverse effects on the fetus
- Causes a delay in parturition in animals
- Associated with premature closure of ductus arteriosus and severe pulmonary hypertension in infants
- Balancing benefits to the mother against possible risks to the fetus
- Excreted in small amounts in breast milk for nursing mothers
Precautions & Warnings
- Long-term administration leading to renal papillary necrosis
- Reports of acute interstitial nephritis, hematuria, proteinuria
- Elimination primarily by the kidneys
- Use with caution in patients with impaired renal function
- Monitoring of serum creatinine and/or creatinine clearance advised
- Reports of moderate to severe jaundice attributed to Naprosyn
