Pharmacology

  • Block COX-1 and COX-2 enzymes
  • Decrease prostaglandin synthesis
  • Offer antipyretic, analgesic, and anti-inflammatory properties
  • Linked to gastrointestinal upset and renal toxicities

Dosage & Administration

  • Taken with water after meals
  • Adult dosage for different indications
  • Children's dosage for specific conditions
  • Naproxen Gel application frequency

Interaction

  • Diminish ACE inhibitors' antihypertensive effect
  • Delay absorption with antacids & Sucralfate
  • Increase adverse effects when taken with Aspirin
  • Reduce natriuretic effect of Diuretics
  • Enhance Methotrexate toxicity
  • Increase the risk of GI bleeding with Warfarin
  • Increase the risk of GI bleeding with Selective Serotonin Reuptake Inhibitors (SSRI)

Contraindications

  • Known hypersensitivity to Naproxen
  • Experienced asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
  • Treatment of perioperative pain in the setting of coronary artery bypass graft (CABG) surgery

Side Effects

  • Gastrointestinal: Heartburn, abdominal pain, nausea, diarrhea, dyspepsia
  • Central Nervous System: Headache, vertigo, drowsiness
  • Dermatological: Pruritus (itching), purpura
  • Cardiovascular: Edema, palpitation
  • Others: Visual disturbances, hearing disturbances

Pregnancy & Lactation

  • Adverse effect on the fetus through pharmacological properties
  • Causes a delay in parturition and associated with premature closure of the ductus arteriosus and severe pulmonary hypertension in infants born to mothers taking Naproxen
  • Requires careful balancing of benefits to the mother against possible risks to the fetus
  • Excreted in small amounts in breast milk for nursing mothers
  • Amount distributed into breast milk is considered by some authorities to be too small to be harmful to a breast-fed infant

Precautions & Warnings

  • Long-term administration resulted in renal papillary necrosis and other abnormal renal pathology in animals
  • Reports of acute interstitial nephritis, hematuria, proteinuria, and occasionally nephrotic syndrome associated with Naprox-containing products
  • Eliminated primarily by the kidneys
  • Use with caution in patients with significantly impaired renal function
  • Caution if given to patients with creatinine clearance of less than 20ml/min
  • Reports of moderate to severe jaundice attributed to Naprox
