Indications

Pharmacology

  • Blocks COX-1 and COX-2 enzymes
  • Decreases prostaglandin synthesis
  • Offers antipyretic, analgesic, and anti-inflammatory properties
  • Linked to gastrointestinal upset and renal toxicities

Dosage & Administration

  • Taken with water, preferably after meals
  • For adults: 500-1000 mg per day in two divided doses at 12-hour intervals
  • For acute gout: 750 mg initially, then 250 mg every 8 hours
  • For dysmenorrhoea: 500 mg initially, followed by 250 mg at 6-8 hour intervals for up to 5 days
  • For children over 2 years: 10 mg/kg/day in two divided doses at 12-hour intervals
  • Naproxen Gel: Applied 2-6 times a day as required

Interaction

  • Diminishes the antihypertensive effect of ACE inhibitors
  • Delays the absorption of Napryn
  • Increases adverse effects when taken with aspirin
  • Reduces the natriuretic effect of diuretics
  • Enhances the toxicity of Methotrexate
  • Increases the risk of GI bleeding with Warfarin and Selective Serotonin Reuptake Inhibitors (SSRI)

Contraindications

  • Patients with known hypersensitivity to Naproxen
  • Patients who have experienced asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
  • Treatment of perioperative pain in the setting of coronary artery bypass graft (CABG) surgery

Side Effects

  • Gastrointestinal: Heartburn, abdominal pain, nausea, diarrhea, dyspepsia
  • Central Nervous System: Headache, vertigo, drowsiness
  • Dermatological: Pruritus (itching), purpura
  • Cardiovascular: Edema, palpitation
  • Others: Visual disturbances, hearing disturbances

Pregnancy & Lactation

  • Adverse effects on the fetus
  • Causes a delay in parturition in animals
  • Associated with premature closure of the ductus arteriosus and severe pulmonary hypertension in infants born to mothers taking Naproxen
  • Excreted in small amounts in breast milk
  • Considered by some authorities to be too small to be harmful to a breastfed infant

Precautions & Warnings

  • Long-term administration may result in renal papillary necrosis and other abnormal renal pathology
  • Reports of acute interstitial nephritis, hematuria, proteinuria, and occasionally nephrotic syndrome
  • Use with caution in patients with significantly impaired renal function
  • Monitoring of serum creatinine and/or creatinine clearance advised
  • Few reports of moderate to severe jaundice attributed to Napryn
