Description

  • Naproxen is indicated for the relief of signs and symptoms of rheumatoid arthritis, osteoarthritis, ankylosing spondylitis, juvenile arthritis, tendonitis, bursitis, acute gout, and primary dysmenorrhea & pain. It exerts its clinical effects by blocking COX-1 and COX-2 enzymes leading to decreased prostaglandin synthesis. The dosage and administration vary for different conditions and age groups. It interacts with ACE inhibitors, antacids & sucralfate, aspirin, diuretics, methotrexate, warfarin, and selective serotonin reuptake inhibitors (SSRI). It is contraindicated in patients with known hypersensitivity to Naproxen and in the setting of coronary artery bypass graft (CABG) surgery. Common side effects include gastrointestinal discomfort, central nervous system effects, dermatological effects, cardiovascular effects, visual and hearing disturbances. Use during pregnancy and lactation requires careful consideration of benefits and risks. Precautions should be taken for renal and hepatic effects. It belongs to the therapeutic class of drugs for osteoarthritis, rheumatoid arthritis, and non-steroidal anti-inflammatory drugs (NSAIDs). It should be stored below 30°C, protected from light and moisture, and kept out of the reach of children.

Pharmacology

  • Blocks COX-1 and COX-2 enzymes leading to decreased prostaglandin synthesis
  • COX-2 mediates the antipyretic, analgesic, and anti-inflammatory properties offered by Naproxen
  • COX-1 enzyme is linked to undesired adverse effects such as gastrointestinal upset and renal toxicities

Dosage instructions

  • Take with water, preferably after meals
  • Dosage varies for rheumatoid arthritis, osteoarthritis, ankylosing spondylitis, acute gout, dysmenorrhoea, juvenile rheumatoid arthritis, analgesia and acute musculoskeletal disorders

Interaction

  • Diminishes the antihypertensive effect of ACE inhibitors
  • Delays the absorption of Xenapro when taken with antacids & sucralfate
  • Increases adverse effects when taken with aspirin
  • Reduces the natriuretic effect of diuretics
  • Enhances the toxicity of Methotrexate
  • Increases the risk of GI bleeding when taken with warfarin or selective serotonin reuptake inhibitors (SSRI)

Contraindications

  • Known hypersensitivity to Naproxen
  • Asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
  • Treatment of perioperative pain in the setting of coronary artery bypass graft (CABG) surgery

Side effects

  • Gastrointestinal discomfort: heartburn, abdominal pain, nausea, diarrhea, dyspepsia
  • Central nervous system effects: headache, vertigo, drowsiness
  • Dermatological effects: pruritus, purpura
  • Cardiovascular effects: edema, palpitation
  • Visual and hearing disturbances

Pregnancy and lactation

  • May have adverse effects on the fetus
  • Causes a delay in parturition in animals
  • Associated with premature closure of the ductus arteriosus and severe pulmonary hypertension in infants born to mothers taking Naproxen
  • Excreted in small amounts in breast milk
  • Use in the first and third trimesters requires careful balancing of benefits to the mother against possible risks to the fetus
  • Some authorities recommend avoiding breastfeeding during Naproxen therapy

Precautions and warnings

  • Long-term administration may result in renal papillary necrosis and abnormal renal pathology
  • Reports of acute interstitial nephritis, hematuria, proteinuria and occasionally nephrotic syndrome
  • Caution in patients with significantly impaired renal function
  • Monitoring of serum creatinine and/or creatinine clearance is advised
  • Reports of moderate to severe jaundice attributed to Xenapro
