Optafenac 0.1% (Ophthalmic Suspension)

Indications

• Treatment of post-operative ocular pain and inflammation
• Inhibition of surgery induced miosis
• Prevention of post-operative cystoid macular edema (CME)

Pharmacology

• Nonsteroidal anti-inflammatory prodrug (NSAID)
• Penetrates the cornea
• Converted to Amfenac by ocular tissue hydrolase
• Inhibits cyclooxygenase enzyme
• Required for prostaglandin synthesis

Dosage & Administration

• Nepafenac 0.1% Sterile Ophthalmic Suspension
• Nepafenac 0.3% Sterile Ophthalmic Suspension
• Instill 1 drop 3 times daily 1 day prior to cataract surgery
• Instill 1 drop once daily 1 day prior to cataract surgery
• Continued on the day of surgery and through the post-operative period
• Additional drop to be administered 30 to 120 minutes prior to surgery

Contraindications

• Hypersensitivity to any of the ingredients
• Hypersensitivity to other NSAIDs

Side Effects

• Foreign body sensation
• Lid margin crusting
• Ocular discomfort
• Ocular hyperemia

Pregnancy & Lactation

• Caution in pregnancy
• Caution in lactation
• Use only if potential benefit justifies potential risk to the fetus

Precautions & Warnings

• Use with caution in patients with known bleeding tendencies
• Use with caution in patients receiving medications which may prolong bleeding time

Use in Special Populations

• Safety and effectiveness in pediatric patients below 18 years not established
• No overall differences in safety or effectiveness between elderly and younger patients
