Indications
- Relief of signs and symptoms of osteoarthritis
- Relief of signs and symptoms of rheumatoid arthritis
- Relief of signs and symptoms of ankylosing spondylitis
- Decrease the risk of developing gastric ulcers in patients at risk of NSAID-associated gastric ulcers
Pharmacology
- Immediate release Esomeprazole Magnesium layer
- Enteric-coated Naproxen core
- Mechanism of action to inhibit prostaglandin synthesis
Dosage
- Adult dosage of 375 mg naproxen/20 mg esomeprazole
- Adult dosage of 500 mg naproxen/20 mg esomeprazole
- Adolescent dosage based on weight
Administration
- To be taken at least 30 minutes before meals
- Should not be split, chewed, crushed or dissolved
Interaction
- Concomitant use with ACE inhibitors, diuretics, and beta-blockers
- Concomitant use with warfarin
- Interference with absorption of drugs where gastric pH is important
- Effect of high-fat food on absorption
Contraindications
- Known hypersensitivity to any component of the tablet
- History of allergic reactions after taking aspirin or other NSAIDs
- Use during the peri-operative period in the setting of coronary artery bypass graft (CABG) surgery
Side Effects
- Decrease in occurrence of gastric ulcers and NSAID associated upper gastrointestinal adverse events
- Potential association with arterial thrombotic events
- Reported adverse events including gastrointestinal effects, oedema, hypertension, and cardiac failure
Pregnancy & Lactation
- Contraindicated during the third trimester of pregnancy
- Not recommended for breastfeeding mothers
- Potential impact on female fertility
Precautions & Warnings
- Avoid combination with NSAIDs and cyclooxygenase-2 selective inhibitors
- Minimum effective dose for the shortest duration necessary
- Risk factors for NSAID related gastro-intestinal complications
- Increased frequency of adverse reactions in older people
- GI bleeding, ulceration or perforation risks
Use in Special Populations
- Increased unbound plasma fraction in elderly
- Not recommended in moderate to severe renal impairment
- Monitor patients with hepatic insufficiency closely
