Medicine Details

Indications

  • Osteoarthritis
  • Rheumatoid arthritis
  • Ankylosing spondylitis
  • Decrease risk of developing gastric ulcers

Pharmacology

  • Immediate release Esomeprazole Magnesium layer
  • Enteric-coated Naproxen core
  • NSAID with analgesic & antipyretic properties
  • Proton pump inhibitor that suppresses gastric acid secretion

Dosage

  • Use lowest effective dose
  • Adult dosage: 375 mg naproxen/20 mg esomeprazole or 500 mg naproxen/20 mg esomeprazole twice daily
  • Adolescent dosage: Based on weight
  • Do not split, chew, crush or dissolve the tablet

Administration

  • Take at least 30 minutes before meals

Interaction

  • Reduced antihypertensive effect of ACE inhibitors, diuretics & beta-blockers with concomitant use of NSAIDs
  • Increased risk of bleeding when used with warfarin
  • Inhibition of gastric acid secretion & interference with drug absorption by Esomeprazole
  • Effect of high-fat food on absorption of naproxen & esomeprazole

Contraindications

  • Known hypersensitivity to any component
  • History of asthmay urticaria or other allergic-type reactions after taking aspirin or other NSAIDs
  • Use during the peri-operative period in the setting of coronary artery bypass graft (CABG) surgery

Side Effects

  • Decreased occurrence of gastric ulcers and NSAID associated upper gastrointestinal adverse events
  • Risk of arterial thrombotic events with high doses & long-term treatment
  • Reported adverse events include gastrointestinal issues, oedema, hypertension, and cardiac failure

Pregnancy & Lactation

  • Not recommended during third trimester of pregnancy
  • Unknown excretion of esomeprazole in human milk
  • Potential impact on fertility

Precautions & Warnings

  • Avoid combination with other NSAIDs
  • Minimize undesirable effects by using lowest effective dose
  • Consider protective agents for certain patients
  • Increased risk of gastrointestinal infections with proton pump inhibitors
  • Possible reduction in vitamin B12 absorption
  • Monitor hypertension and heart failure patients closely

Use in Special Populations

  • Dosage adjustments for elderly and patients with renal impairment
  • Not recommended in patients with severe hepatic impairment
