Indications

  • Relief of signs and symptoms of osteoarthritis
  • Relief of signs and symptoms of rheumatoid arthritis
  • Relief of signs and symptoms of ankylosing spondylitis
  • Decrease the risk of developing gastric ulcers in patients at risk of developing NSAID-associated gastric ulcers

Pharmacology

  • Consists of an immediate release Esomeprazole Magnesium layer and an enteric-coated Naproxen core
  • Naproxen is a NSAID with analgesic and antipyretic properties
  • Esomeprazole is a proton pump inhibitor that suppresses gastric acid secretion

Dosage

  • Use the lowest effective dose for the shortest duration consistent with individual patient treatment goals
  • Adult dosing: 375 mg naproxen/20 mg of esomeprazole twice daily or 500 mg naproxen/20 mg of esomeprazole twice daily
  • Juvenile dosing (weight-based): 375 mg naproxen/20 mg of esomeprazole or 500 mg naproxen/20 mg of esomeprazole for weight greater than 50 kg, and 375 mg naproxen/20 mg of esomeprazole for weight 38 kg to less than 50 kg

Administration

  • Do not split, chew, crush or dissolve the tablet
  • Take at least 30 minutes before meals

Interaction

  • Concomitant use of NSAIDs may reduce the antihypertensive effect of ACE inhibitors, diuretics, and beta-blockers
  • Concomitant use of this tablet and warfarin may result in an increased risk of a bleeding complication
  • Esomeprazole may interfere with the absorption of drugs where gastric pH is an important determinant of bioavailability
  • Administration with high-fat food does not affect the extent of absorption of naproxen but significantly prolongs tmax and decreases peak plasma concentration (Cmax)

Contraindications

  • Known hypersensitivity to any component of this tablet or substituted benzimidazoles
  • History of asthma, urticaria, or other allergic-type reactions after taking aspirin or other NSAIDs
  • Use during the peri-operative period in the setting of coronary artery bypass graft (CABG) surgery

Side Effects

  • Decrease in occurrence of gastric ulcers and NSAID associated upper gastrointestinal adverse events compared to Naproxen alone
  • Possible risk of arterial thrombotic events, oedema, hypertension, and cardiac failure
  • Commonly observed adverse events include gastrointestinal issues such as peptic ulcers, bleeding, nausea, vomiting, and diarrhoea

Pregnancy & Lactation

  • Not recommended during the third trimester of pregnancy
  • Should not be used during breastfeeding
  • Use may impair female fertility and is not recommended in women attempting to conceive

Precautions & Warnings

  • Avoid use with other NSAIDs including cyclooxygenase-2 selective inhibitors
  • Minimize undesirable effects by using the lowest effective dose for the shortest duration necessary to control symptoms
  • Elderly and patients with renal, hepatic, or cardiovascular issues should be monitored closely during treatment

Use in Special Populations

  • Use caution and possible dosage adjustment in elderly patients
  • Not recommended for use in patients with moderate to severe renal impairment
  • Monitor closely and consider possible dose reduction in patients with mild to moderate hepatic impairment

Overdose Effects

  • Symptoms may include lethargy, dizziness, epigastric pain, abdominal discomfort, heartburn, and more
  • Management may involve symptomatic and supportive care, and there are no specific antidotes for overdose
