Indications
- Relief of signs and symptoms of osteoarthritis
- Relief of signs and symptoms of rheumatoid arthritis
- Relief of signs and symptoms of ankylosing spondylitis
- Decrease the risk of developing gastric ulcers in patients at risk of developing NSAID-associated gastric ulcers
Pharmacology
- Immediate release Esomeprazole Magnesium layer
- Enteric-coated Naproxen core
- Esomeprazole is released first into the stomach
- Naproxen is dissolved in the small intestine
- Naproxen is a NSAID with analgesic and antipyretic properties
- Naproxen inhibits prostaglandin synthesis
- Esomeprazole is a proton pump inhibitor
- Esomeprazole suppresses gastric acid secretion
- Esomeprazole specifically inhibits the H+/k+ -ATPase in the gastric parietal cell
- Esomeprazole blocks the final step in acid production
Dosage
- Use the lowest effective dose for the shortest duration
- Recommended dosage for adults with rheumatoid arthritis, osteoarthritis, and ankylosing spondylitis
- Recommended dosage for juvenile idiopathic arthritis in adolescent patients
- Different treatment should be considered if a lower dose of Esomeprazole is appropriate
Administration
- Do not split, chew, crush or dissolve the tablet
- Take the tablet at least 30 minutes before meals
Interaction
- Concomitant use of NSAIDs may reduce the antihypertensive effect of ACE inhibitors, diuretics, and beta-blockers
- Concomitant use of this tablet and warfarin may result in an increased risk of a bleeding complication
- Esomeprazole may interfere with the absorption of drugs where gastric pH is an important determinant of bioavailability
- Administration of Naproxen and Esomeprazole together with high-fat food may affect the absorption of naproxen
Contraindications
- Known hypersensitivity to any component of the tablet or substituted benzimidazoles
- History of asthma, urticaria, or other allergic-type reactions after taking aspirin or other NSAIDs
- Use during the peri-operative period in the setting of coronary artery bypass graft (CABG) surgery
Side Effects
- Decreased occurrence of gastric ulcers and NSAID associated upper gastrointestinal adverse events compared to Naproxen alone
- Possible association with a small increased risk of arterial thrombotic events
- Reported adverse events include gastrointestinal issues, oedema, hypertension, and cardiac failure
Pregnancy & Lactation
- Not recommended during the first and second trimester of pregnancy
- Contraindicated during the third trimester of pregnancy
- Not recommended during breastfeeding
- Use may impair female fertility
Precautions & Warnings
- Avoid combination with NSAIDs including cyclooxygenase-2 selective inhibitors
- Use caution in patients with high age, concomitant use of anticoagulants, corticosteroids, or other NSAIDs
- Increased risk of adverse reactions in older people
- Gastrointestinal bleeding, ulceration, or perforation may occur
- Special consideration in patients with conditions like Inducible porphyries, Systemic lupus erythematosus and mixed connective tissue disease
- Appropriate monitoring and advice required for patients with a history of hypertension and/or mild to moderate congestive heart failure
- Long-term administration may result in renal papillary necrosis and other renal injury
Use in Special Populations
- Caution required in elderly patients
- Not recommended for patients with moderate to severe renal impairment
- Monitor patients with mild to moderate hepatic impairment closely
