Indications
- Relief of signs and symptoms of osteoarthritis
- Relief of signs and symptoms of rheumatoid arthritis
- Relief of signs and symptoms of ankylosing spondylitis
- Reducing the risk of developing gastric ulcers in patients at risk of NSAID-associated gastric ulcers
Pharmacology
- Consists of immediate release Esomeprazole Magnesium layer
- Enteric-coated Naproxen core
- Naproxen is an NSAID with analgesic and antipyretic properties
- Mechanism of action of Naproxen is to inhibit prostaglandin synthesis
- Esomeprazole is a proton pump inhibitor that suppresses gastric acid secretion by specific inhibition of the H+/k+ -ATPase in the gastric parietal cell
- Reduces gastric acidity
Dosage
- Use the lowest effective dose
- Adult dosage: One tablet twice daily of either 375mg naproxen/20mg esomeprazole or 500mg naproxen/20mg esomeprazole
- Juvenile dosage: Based on weight
Administration
- Take at least 30 minutes before meals
- Do not split, chew, crush, or dissolve the tablet
Interaction
- Concomitant use of NSAIDs may reduce the antihypertensive effect of ACE inhibitors, diuretics, and beta-blockers
- Concomitant use with warfarin may result in increased risk of bleeding
- Inhibition of drug absorption by Esomeprazole
Contraindications
- Known hypersensitivity to any component of the tablet or substituted benzimidazoles
- History of asthmatic or allergic-type reactions after taking aspirin or other NSAIDs
- Use during peri-operative period in the setting of coronary artery bypass graft (CABG) surgery
Side effects
- Decreased occurrence of gastric ulcers and NSAID associated upper gastrointestinal adverse events compared to Naproxen alone
- Possible side effects include gastrointestinal issues, arterial thrombotic events, oedema, hypertension, cardiac failure
Pregnancy and lactation
- Not recommended during the third trimester of pregnancy
- Not recommended during breastfeeding
- May impair female fertility
Precautions and warnings
- Avoid use of Naproxen and NSAIDs together
- Minimize undesirable effects by using the lowest effective dose
- Special caution for older people
- Increased risk of gastrointestinal bleeding and perforation
- Monitoring and advice required for patients with a history of hypertension and/or congestive heart failure
- Potential impact on renal function
Use in special populations
- Caution required in elderly patients
- Not recommended for patients with moderate to severe or severe renal impairment
- Close monitoring and possible dose reduction in patients with mild to moderate hepatic impairment
Overdose effects
- Symptoms of Naproxen overdose such as lethargy, dizziness, gastrointestinal issues, renal dysfunction
- Forced diuresis, alkalinization of urine may not be useful for Naproxen overdose
- No specific antidote known for Esomeprazole overdose
