Indications

  • Relief of osteoarthritis
  • Relief of rheumatoid arthritis
  • Relief of ankylosing spondylitis
  • Decrease risk of developing gastric ulcers

Mode of Action

  • Immediate release Esomeprazole Magnesium layer
  • Enteric-coated Naproxen core
  • Inhibition of prostaglandin synthesis

Dosage

  • Use lowest effective dose
  • Adult dosage: 375 mg naproxen/20 mg esomeprazole, or 500 mg naproxen/20 mg esomeprazole twice daily
  • Adolescent dosage: 375 mg naproxen/20 mg esomeprazole, or 500 mg naproxen/20 mg esomeprazole based on weight

Administration

Take at least 30 minutes before meals

Interactions

  • Reduced antihypertensive effect with NSAIDs
  • Increased bleeding risk with warfarin
  • Potential interference with drug absorption (e.g., ketoconazole, iron salts, digoxin)
  • Effect of high-fat food on absorption

Contraindications

  • Known hypersensitivity to the tablet components
  • History of allergic reactions after taking aspirin or NSAIDs
  • Use during peri-operative period in setting of CABG surgery

Side Effects

  • Decreased occurrence of gastric ulcers
  • Potential risk of arterial thrombotic events
  • Gastrointestinal adverse events
  • Oedema, hypertension, and cardiac failure
  • Nausea, vomiting, diarrhea, constipation, abdominal pain, and more

Pregnancy & Lactation

  • Avoid use during first and second trimester of pregnancy
  • Contraindicated in third trimester
  • Avoid use during breastfeeding
  • Potential impact on fertility

Precautions & Warnings

  • Avoid use with other NSAIDs
  • Use with low dose acetylsalicylic acid
  • Minimize undesirable effects with lowest effective dose
  • Regular surveillance for long-term treatment
  • Increased frequency of adverse reactions in older people
  • Consider combination therapy for patients with history of GI toxicity

Use in Special Populations

  • Not recommended in patients with moderate to severe renal impairment
