Medicine Details

Indications

  • Relief of signs and symptoms of osteoarthritis
  • Relief of signs and symptoms of rheumatoid arthritis
  • Relief of signs and symptoms of ankylosing spondylitis
  • Decrease the risk of developing gastric ulcers in patients at risk of developing NSAID-associated gastric ulcers

Pharmacology

  • Immediate release Esomeprazole Magnesium layer
  • Enteric-coated Naproxen core
  • Inhibition of prostaglandin synthesis
  • Suppression of gastric acid secretion by specific inhibition of the H+/k+ -ATPase
  • Reduction of gastric acidity

Dosage

  • Use the lowest effective dose for the shortest duration consistent with individual patient treatment goals

Administration

  • Take at least 30 minutes before meals

Interaction

  • Reduction of antihypertensive effect of ACE inhibitors, diuretics & beta-blockers
  • Increased risk of bleeding complication with warfarin
  • Interference with the absorption of drugs where gastric pH is important
  • Prolongation of absorption time and decrease in peak plasma concentration with high-fat food

Contraindications

  • Known hypersensitivity to any component of the tablet or substituted benzimidazoles
  • History of asthmay urticaria, or other allergic-type reactions after taking aspirin or other NSAIDs
  • Use during the peri-operative period in the setting of coronary artery bypass graft (CABG) surgery

Side Effects

  • Decrease in occurrence of gastric ulcers and NSAID associated upper gastrointestinal adverse events
  • Potential risk of arterial thrombotic events with high doses and long-term treatment
  • Gastrointestinal adverse events such as nausea, vomiting, diarrhoea, and abdominal pain

Pregnancy & Lactation

  • Not recommended during the third trimester of pregnancy
  • Not recommended during breastfeeding
  • Impairment of female fertility

Precautions & Warnings

  • Avoid combination with other NSAIDs
  • Minimize undesirable effects by using the lowest effective dose
  • Consider risk-factors and regular surveillance for gastrointestinal complications
  • Increased frequency of adverse reactions in older people
  • Appropriate monitoring for patients with hypertension and congestive heart failure
  • Risk of renal papillary necrosis and other renal injury with long-term administration

Use in Special Populations

  • Adjustment of dosage in elderly patients
  • Not recommended for use in patients with moderate to severe renal impairment
  • Close monitoring in patients with mild to moderate hepatic impairment
  • Not recommended in patients with severe hepatic impairment

Overdose Effects

  • Symptoms including lethargy, dizziness, abdominal discomfort, and renal dysfunction
  • Management by symptomatic and supportive care
  • No specific antidote known
