Indications
- Relief of signs & symptoms of Osteoarthritis
- Relief of signs & symptoms of Rheumatoid arthritis
- Relief of signs & symptoms of Ankylosing spondylitis
- Decrease the risk of developing gastric ulcers in patients at risk of developing NSAID-associated gastric ulcers
Pharmacology
- Consists of immediate release Esomeprazole Magnesium layer & enteric-coated Naproxen core
- Naproxen is a NSAID with analgesic & antipyretic properties
- Esomeprazole is a proton pump inhibitor that suppresses gastric acid secretion by specific inhibition of the H+/k+ -ATPase in the gastric parietal cell
Dosage
- Adults: One tablet twice daily of 375 mg naproxen/20 mg of esomeprazole or 500 mg naproxen/20 mg of esomeprazole
- Juvenile Idiopathic Arthritis in Adolescent Patients 12 Years of Age & Older: 375 mg naproxen/20 mg of esomeprazole or 500 mg naproxen/20 mg of esomeprazole for weight greater than 50 kg, 375 mg naproxen/20 mg of esomeprazole for weight 38 kg to less than 50 kg
Administration
- Tablet is to be taken at least 30 minutes before meals
Interaction
- Concomitant use of NSAIDs may reduce the antihypertensive effect of ACE inhibitors, diuretics & beta-blockers
- Increased risk of bleeding complication with warfarin
- Potential interference with the absorption of drugs like Ketoconazole, iron salts and digoxin
- Effects of administration with high-fat food on absorption of naproxen
Contraindications
- Known hypersensitivity to any component of the tablet or substituted benzimidazoles
- History of asthmay urticaria or other allergic-type reactions after taking aspirin or other NSAIDs
- Use during the peri-operative period in the setting of coronary artery bypass graft (CABG) surgery
Side Effects
- Decrease in occurrence of gastric ulcers and NSAID associated upper gastrointestinal adverse events compared to Naproxen alone
- Possible association with risk of arterial thrombotic events
- Adverse events include gastrointestinal symptoms, oedema, hypertension, cardiac failure
Pregnancy & Lactation
- Not recommended during the third trimester of pregnancy
- Not recommended during breastfeeding
- Potential impairment of female fertility
Precautions & Warnings
- Risk factors for developing NSAID related gastro-intestinal complications
- Increased frequency of adverse reactions in older people
- Risk of GI bleeding, ulceration or perforation with NSAIDs
- Monitoring required for patients with history of hypertension and/or mild to moderate congestive heart failure
Use in Special Populations
- Caution and dosage adjustment in elderly patients
- Not recommended for use in patients with moderate to severe or severe renal impairment
- Close monitoring in patients with hepatic insufficiency
Overdose Effects
- Symptoms related to Naproxen overdose
- Transient symptoms related to esomeprazole overdose
- No specific antidotes for either component
- Use of symptomatic and supportive care
