Dosage and Administration
- Recommended Dosage: One tablet twice daily
- Adult Dosage: 375 mg naproxen/20 mg esomeprazole or 500 mg naproxen/20 mg esomeprazole
- Adolescent Dosage (Weight > 50 kg): 375 mg naproxen/20 mg esomeprazole or 500 mg naproxen/20 mg esomeprazole
- Adolescent Dosage (Weight 38 kg to < 50 kg): 375 mg naproxen/20 mg esomeprazole
- Administration: Take at least 30 minutes before meals
- Do not split, chew, crush, or dissolve the tablet
Indications
- Relief of Osteoarthritis signs and symptoms
- Relief of Rheumatoid Arthritis signs and symptoms
- Relief of Ankylosing Spondylitis signs and symptoms
- Decrease in risk of developing gastric ulcers
Pharmacology
- Immediate release Esomeprazole Magnesium layer
- Enteric-coated Naproxen core
- Inhibition of prostaglandin synthesis
- Suppression of gastric acid secretion
Interaction
- Concomitant use of NSAIDs may reduce antihypertensive effect of ACE inhibitors, diuretics, and beta-blockers
- Concomitant use with warfarin may increase risk of bleeding
- Inhibition of gastric acid secretion may interfere with drug absorption
- Administration with high-fat food prolongs time to reach peak plasma concentration
Contraindications
- Known hypersensitivity to tablet components
- History of allergic-type reactions to aspirin or other NSAIDs
- Use during peri-operative period in coronary artery bypass graft (CABG) surgery
Side Effects
- Decreased occurrence of gastric ulcers and NSAID-associated upper gastrointestinal adverse events
- Possible increased risk of arterial thrombotic events
- Reported adverse events: hypertension, cardiac failure, GI bleeding, nausea, vomiting, diarrhea, constipation, dyspepsia, abdominal pain
Pregnancy and Lactation
- Not recommended during third trimester of pregnancy
- Should not be used during breastfeeding
- May impair female fertility
Precautions and Warnings
- Avoid use with other NSAIDs
- Use lowest effective dose for shortest duration
- Patients with cardiovascular or cerebrovascular diseases should use with caution
- Long-term treatment requires regular surveillance
Use in Special Populations
- Use caution and possible dosage adjustment in elderly patients
- Not recommended for patients with moderate to severe renal impairment
- Monitor patients with mild to moderate hepatic impairment closely
Overdose Effects
- Symptoms: lethargy, dizziness, abdominal discomfort, renal dysfunction, hypertension
- No specific antidotes, manage with supportive care
