Indications

  • Relief of signs and symptoms of Osteoarthritis
  • Relief of signs and symptoms of Rheumatoid arthritis
  • Relief of signs and symptoms of Ankylosing spondylitis
  • Decrease the risk of developing gastric ulcers in patients at risk of developing NSAID-associated gastric ulcers

Pharmacology

  • Consists of an immediate release Esomeprazole Magnesium layer
  • Enteric-coated Naproxen core
  • Esomeprazole is released first into the stomach
  • Naproxen is released in the small intestine
  • Naproxen is a NSAID with analgesic and antipyretic properties
  • Inhibits the prostaglandin synthesis
  • Esomeprazole is a proton pump inhibitor
  • Suppresses gastric acid secretion
  • Specific inhibition of the H+/k+ -ATPase in the gastric parietal cell
  • Blocks the final step in acid production

Dosage

  • Use the lowest effective dose for the shortest duration
  • Adults dose: One tablet twice daily of either: 375 mg naproxen/20 mg of esomeprazole; or 500 mg naproxen/20 mg of esomeprazole
  • Juvenile Idiopathic Arthritis dose: Depends on weight

Administration

  • Do not split, chew, crush or dissolve the tablet
  • Taken at least 30 minutes before meals

Interaction

  • Concomitant use of NSAIDs may reduce the antihypertensive effect of ACE inhibitors, diuretics, and beta-blockers
  • Warfarin interaction may result in increased risk of a bleeding complication
  • Inhibits the absorption of drugs where gastric pH is an important determinant of bioavailability
  • Administration with high-fat food affects absorption of naproxen

Contraindications

  • Known hypersensitivity to any component of this tablet or substituted benzimidazoles
  • History of asthmay urticaria or other allergic-type reactions after taking aspirin or other NSAIDs
  • Use during the peri-operative period in the setting of coronary artery bypass graft (CABG) surgery

Side Effects

  • Decrease the occurrence of gastric ulcers and NSAID associated upper gastrointestinal adverse events
  • Increased risk of arterial thrombotic events
  • Gastrointestinal adverse events
  • Oedema, hypertension, and cardiac failure
  • Nausea, vomiting, diarrhea, flatulence, constipation, dyspepsia, abdominal pain
  • Peptic ulcers, perforation or GI bleeding
  • Gastritis
  • Exacerbation of colitis and Crohn's disease

Pregnancy & Lactation

  • Not recommended during the third trimester of pregnancy
  • Should not be used during breastfeeding
  • May impair female fertility

Precautions & Warnings

  • Avoid combination with NSAIDs including cyclooxygenase-2 selective inhibitors
  • Use with low dose acetylsalicylic acid
  • Minimize undesirable effects by using the lowest effective dose
  • Risk-factors for developing NSAID related gastro-intestinal complications
  • Increased frequency of adverse reactions in older people
  • May exacerbate gastrointestinal disease
  • Caution in patients with a history of gastrointestinal disease
  • Appropriate monitoring for patients with a history of hypertension and/or mild to moderate congestive heart failure
  • Consideration for patients with risk factors for cardiovascular events
  • Monitor patients with hepatic and renal impairment closely

Use in Special Populations

  • Naproxen unbound plasma fraction is increased in the elderly
  • Not recommended for use in patients with moderate to severe or severe renal impairment
  • Monitor patients with mild to moderate hepatic impairment closely
  • Not recommended in patients with severe hepatic impairment

Overdose Effects

  • Symptoms related to Naproxen overdose
  • Symptoms related to Esomeprazole overdose
  • Management involves symptomatic and supportive care
