Medicine Details

Indications

  • Relief of signs and symptoms of Osteoarthritis
  • Relief of signs and symptoms of Rheumatoid arthritis
  • Relief of signs and symptoms of Ankylosing spondylitis
  • Decrease the risk of developing gastric ulcers

Pharmacology

  • Immediate release Esomeprazole Magnesium layer
  • Enteric-coated Naproxen core
  • Esomeprazole released first into the stomach
  • Naproxen dissolves in the small intestine
  • Naproxen inhibits prostaglandin synthesis
  • Esomeprazole suppresses gastric acid secretion

Dosage

  • Use lowest effective dose
  • Adult dosage: 375 mg naproxen/20 mg esomeprazole or 500 mg naproxen/20 mg esomeprazole
  • Adolescent dosage: 375 mg naproxen/20 mg esomeprazole or 500 mg naproxen/20 mg esomeprazole

Administration

  • Take tablet 30 minutes before meals
  • Do not split, chew, crush or dissolve tablet

Interaction

  • Concomitant use of NSAIDs and ACE inhibitors
  • Concomitant use of tablet and warfarin
  • Esomeprazole may interfere with drug absorption
  • Effect of tablet with high-fat food
  • Decreased peak plasma concentration with food

Contraindications

  • Known hypersensitivity to tablet components
  • History of allergic-type reactions to aspirin or NSAIDs
  • Contraindicated during CABG surgery

Side Effects

  • Decreased occurrence of gastric ulcers
  • Potential risk of arterial thrombotic events
  • Gastrointestinal adverse events
  • Possible adverse events: Oedema, hypertension, cardiac failure
  • Commonly observed adverse events: Gastrointestinal issues

Pregnancy & Lactation

  • Use with caution in pregnancy
  • Contraindicated in the third trimester
  • Excretion in human milk unknown
  • Not recommended during breastfeeding
  • NSAIDs may impair female fertility

Precautions & Warnings

  • Avoid combining with other NSAIDs
  • Use with caution in elderly patients
  • Consider risks in patients with various conditions
  • Risk factors for gastrointestinal complications
  • Close monitoring in long-term treatment
  • Increased GI risk in patients with history of ulcers
  • Cardiovascular and cerebrovascular effects
  • Renal effects

Use in Special Populations

  • Unbound plasma fraction increased in elderly
  • Use caution in high doses for elderly patients
  • Not recommended for patients with severe renal impairment
  • Monitor patients with hepatic insufficiency
