Medicine Details

Indications

  • Relief of Osteoarthritis symptoms
  • Relief of Rheumatoid arthritis symptoms
  • Relief of Ankylosing spondylitis symptoms
  • Decrease risk of developing gastric ulcers

Pharmacology

  • Immediate release Esomeprazole Magnesium layer
  • Enteric-coated Naproxen core
  • Esomeprazole is released first into the stomach
  • Naproxen dissolves in the small intestine
  • Naproxen is a NSAID with analgesic properties
  • Naproxen inhibits prostaglandin synthesis
  • Esomeprazole is a proton pump inhibitor
  • Esomeprazole suppresses gastric acid secretion

Dosage

  • Carefully consider potential benefits & risks
  • Use lowest effective dose
  • For Rheumatoid Arthritis, Osteoarthritis and Ankylosing Spondylitis
  • Different doses for adults and juvenile patients

Administration

  • Do not split, chew, crush or dissolve the tablet
  • Take at least 30 minutes before meals

Interaction

  • Concomitant use of NSAIDs may reduce antihypertensive effect
  • Concomitant use with warfarin may increase bleeding risk
  • Esomeprazole may interfere with drug absorption
  • Administration with high-fat food may affect absorption

Contraindications

  • Known hypersensitivity to any component
  • History of asthmay urticaria or allergic-type reactions to aspirin or other NSAIDs
  • Use during peri-operative period in the setting of CABG surgery

Side Effects

  • Decrease in occurrence of gastric ulcers and gastrointestinal adverse events
  • Potential risk of arterial thrombotic events
  • Gastrointestinal adverse events
  • Oedema, hypertension and cardiac failure
  • Nausea, vomiting, diarrhoea, flatulence, constipation, dyspepsia

Pregnancy & Lactation

  • Not recommended during first and second trimester
  • Contraindicated during third trimester
  • Not recommended during breastfeeding
  • May impair female fertility

Precautions & Warnings

  • Avoid combination with other NSAIDs
  • Use with low dose acetylsalicylic acid
  • Minimize undesirable effects by using lowest effective dose
  • Monitor high-risk patients closely
  • Consider protective agents for high-risk patients

Use in Special Populations

  • Caution in elderly patients
  • Not recommended for patients with moderate to severe renal impairment
  • Monitor patients with hepatic insufficiency closely
