Indications
- Relief of osteoarthritis symptoms
- Relief of rheumatoid arthritis symptoms
- Relief of ankylosing spondylitis symptoms
- Reduction of risk of developing gastric ulcers
Pharmacology
- Consists of immediate release Esomeprazole Magnesium layer
- Consists of enteric-coated Naproxen core
- Esomeprazole released first into stomach
- Dissolution of Naproxen in small intestine
- Naproxen: NSAID with analgesic & antipyretic properties
- Mechanism of action of Naproxen: inhibits prostaglandin synthesis
- Esomeprazole: proton pump inhibitor that suppresses gastric acid secretion
Dosage
- Use lowest effective dose for shortest duration
- Recommended dosage for adults
- Recommended dosage for adolescent patients 12 years of age & older
Administration
- Tablet should not be split, chewed, crushed or dissolved
- Take at least 30 minutes before meals
Interaction
- Concomitant use of NSAIDs may reduce antihypertensive effect of ACE inhibitors, diuretics & beta-blockers
- Concomitant use of tablet and warfarin may increase risk of bleeding complication
- Esomeprazole may interfere with absorption of certain drugs
- Administration with high-fat food prolongs time to peak plasma concentration
Contraindications
- Known hypersensitivity to any component of the tablet
- History of asthma, urticaria or allergic-type reactions after taking aspirin or other NSAIDs
- Use during peri-operative period in setting of coronary artery bypass graft (CABG) surgery
Side Effects
- Decreased incidence of gastrointestinal side effects from Naproxen
- Significantly decreased occurrence of gastric ulcers and NSAID associated upper gastrointestinal adverse events
- Possible increased risk of arterial thrombotic events
- Reported adverse events: nausea, vomiting, diarrhoea, ulcerative stomatitis, GI bleeding
- Less frequently observed adverse event: gastritis
- Oedema, hypertension and cardiac failure reported in association with NSAID treatment
Pregnancy & Lactation
- Not recommended during first and second trimester of pregnancy
- Contraindicated during third trimester of pregnancy
- Not recommended during breastfeeding
- May impair female fertility
Precautions & Warnings
- Avoid combination with NSAIDs and cyclooxygenase-2 selective inhibitors
- Use with low dose acetylsalicylic acid
- Minimize undesirable effects by using lowest effective dose for shortest duration
- Risk-factors for developing NSAID related gastro-intestinal complications
- Increased risk of adverse reactions in older people
- Increased risk of GI bleeding, ulceration or perforation
- Appropriate monitoring and advice required for patients with history of hypertension and/or mild to moderate congestive heart failure
- Renal effects: potential renal papillary necrosis and other renal injury
Use in Special Populations
- Studies indicate unbound plasma fraction of Naproxen is increased in the elderly
- Not recommended in patients with moderate or severe renal impairment
- Monitor patients with mild to moderate hepatic impairment closely
Overdose Effects
- Symptoms of Naproxen overdose: lethargy, dizziness, epigastric pain, gastrointestinal bleeding
- Symptoms of esomeprazole overdose: transient symptoms
- Management: symptomatic and supportive care, no specific antidote known
