Medicine Details

Indications

  • Relief of signs & symptoms of Osteoarthritis
  • Relief of signs & symptoms of Rheumatoid arthritis
  • Relief of signs & symptoms of Ankylosing spondylitis
  • Risk reduction of NSAID-associated gastric ulcers

Pharmacology

  • Immediate release Esomeprazole Magnesium layer
  • Enteric-coated Naproxen core
  • NSAID with analgesic & antipyretic properties
  • Inhibition of prostaglandin synthesis
  • Proton pump inhibitor that suppresses gastric acid secretion

Dosage

  • Consideration of potential benefits & risks
  • Use of lowest effective dose
  • Different doses for adults & adolescent patients based on weight

Administration

  • Tablet to be taken at least 30 minutes before meals
  • Not to split, chew, crush or dissolve the tablet

Interaction

  • Reduction of antihypertensive effect of ACE inhibitors, diuretics & beta-blockers
  • Increased risk of bleeding complication with concomitant use of warfarin
  • Interference with absorption of drugs like Ketoconazole, iron salts and digoxin
  • Effect of administration with high-fat food on absorption of naproxen

Contraindications

  • Known hypersensitivity to any component
  • History of asthmay urticaria or other allergic-type reactions after taking aspirin or other NSAIDs
  • Use during the peri-operative period in the setting of coronary artery bypass graft (CABG) surgery

Side Effects

  • Decrease in occurrence of gastric ulcers and NSAID associated upper gastrointestinal adverse events
  • Potential association with arterial thrombotic events
  • Gastrointestinal adverse events like peptic ulcers, perforation or GI bleeding
  • Other adverse events like oedema, hypertension, cardiac failure, naussea, vomiting, diarrhoea, dyspepsia, abdominal pain

Pregnancy & Lactation

  • Contraindication in the third trimester of pregnancy
  • Caution in the first and second trimester
  • Avoidance of use during breastfeeding
  • Possible impairment of female fertility

Precautions & Warnings

  • Avoidance of combination with NSAIDs including cyclooxygenase-2 selective inhibitors
  • Minimization of undesirable effects by using lowest effective dose
  • Risk-factors for developing NSAID related gastro-intestinal complications
  • Increased frequency of adverse reactions in older people
  • Monitoring and advice required for patients with history of hypertension and/or mild to moderate congestive heart failure
  • Effects of long-term administration on renal papillary necrosis and other renal injury

Use in Special Populations

  • Increased unbound plasma fraction of naproxen in elderly
  • Caution and possible dose reduction in elderly patients
  • Non-recommendation for use in patients with moderate to severe or severe renal impairment
  • Monitoring and dose reduction consideration based on hepatic insufficiency
