Indications for Use

  • Relief of signs & symptoms of osteoarthritis
  • Relief of signs & symptoms of rheumatoid arthritis
  • Relief of signs & symptoms of ankylosing spondylitis
  • Decreasing the risk of developing gastric ulcers in patients at risk of developing NSAID-associated gastric ulcers

Pharmacology

  • Consists of an immediate release Esomeprazole Magnesium layer & an enteric-coated Naproxen core
  • Naproxen is a NSAID with analgesic & antipyretic properties
  • Esomeprazole is a proton pump inhibitor that suppresses gastric acid secretion

Dosage

  • Carefully consider potential benefits & risks before use
  • Use the lowest effective dose for the shortest duration consistent with individual patient treatment goals
  • Adult dosage: one tablet twice daily of either 375 mg naproxen/20 mg of esomeprazole, or 500 mg naproxen/20 mg of esomeprazole
  • Adolescent dosage (weight greater than 50 kg): 375 mg naproxen/20 mg of esomeprazole, or 500 mg naproxen/20 mg of esomeprazole
  • Adolescent dosage (weight 38 kg to less than 50 kg): one tablet twice daily of 375 mg naproxen/20 mg of esomeprazole

Administration

  • Do not split, chew, crush, or dissolve the tablet
  • Take at least 30 minutes before meals

Interaction

  • Concomitant use of NSAIDs may reduce the antihypertensive effect of ACE inhibitors, diuretics & beta-blockers
  • Concomitant use of this tablet and warfarin may result in an increased risk of a bleeding complication
  • Esomeprazole inhibits gastric acid secretion & may interfere with the absorption of drugs where gastric pH is important for bioavailability (eg. Ketoconazole, iron salts, digoxin)
  • Administration with high-fat food in healthy volunteers does not affect the extent of absorption of naproxen but significantly prolongs time to reach peak plasma concentration and decreases peak plasma concentration

Contraindications

  • Known hypersensitivity to any component of this tablet or substituted benzimidazoles
  • History of asthma, urticaria, or other allergic-type reactions after taking aspirin or other NSAIDs
  • Use during the peri-operative period in the setting of coronary artery bypass graft (CABG) surgery

Side Effects

  • Includes decreased occurrence of gastric ulcers and NSAID associated upper gastrointestinal adverse events compared to Naproxen alone
  • Risks of arterial thrombotic events associated with long-term, high-dose use
  • Possible adverse events such as oedema, hypertension, cardiac failure, gastrointestinal effects (nausea, vomiting, etc.)

Pregnancy & Lactation

  • Not recommended during first and second trimester of pregnancy unless potential benefit outweighs potential risk
  • Contraindicated during the third trimester of pregnancy
  • Not recommended during breastfeeding

Precautions & Warnings

  • Avoid combination with other NSAIDs
  • Minimize undesirable effects by using lowest effective dose for the shortest duration necessary
  • Increased risk of serious NSAID-related adverse events in older people
  • Consider combination therapy with protective agents for patients with history of ulcer or requiring concomitant low-dose acetylsalicylic acid
  • Caution advised in patients receiving NSAIDs with concomitant medications that could increase risk of ulceration or bleeding
  • Appropriate monitoring and advice required for patients with history of hypertension and/or mild to moderate congestive heart failure
  • Regular surveillance recommended for long-term treatment, especially for those treated for more than a year

Use in Special Populations

  • Use with caution in elderly patients and consider the lowest effective dose
  • Not recommended for use in patients with moderate to severe or severe renal impairment
  • Monitor patients with mild to moderate hepatic impairment closely

Overdose Effects

  • Symptoms of naproxen overdose include lethargy, dizziness, drowsiness, gastrointestinal symptoms, alterations in liver function, and more
  • Symptoms of esomeprazole overdose are transient
  • Management involves symptomatic and supportive care, no specific antidotes
