Indications

  • Osteoarthritis
  • Rheumatoid arthritis
  • Ankylosing spondylitis
  • Decrease the risk of developing gastric ulcers

Pharmacology

  • Immediate release Esomeprazole Magnesium layer
  • Enteric-coated Naproxen core
  • Proton pump inhibitor
  • Inhibit prostaglandin synthesis

Dosage

  • Use lowest effective dose
  • One tablet twice daily for adults
  • Different doses for weight categories in adolescent patients

Administration

  • Take at least 30 minutes before meals

Interaction

  • Reduced antihypertensive effect of ACE inhibitors, diuretics & beta-blockers
  • Increased risk of bleeding with warfarin
  • Interference with absorption of certain drugs
  • Effect of high-fat food on absorption

Contraindications

  • Hypersensitivity to components
  • History of allergic-type reactions to aspirin or other NSAIDs
  • Use during peri-operative period in CABG surgery

Side Effects

  • Decreased incidence of gastrointestinal side effects
  • Lower risk of thrombotic events with Naproxen
  • Gastrointestinal adverse events
  • Oedema, hypertension, cardiac failure
  • Nausea, vomiting, diarrhea, abdominal pain, bleeding

Pregnancy & Lactation

  • Use in first and second trimester only if benefit outweighs risk
  • Avoid during third trimester
  • Avoid during breastfeeding
  • Impaired female fertility with NSAID use

Precautions & Warnings

  • Avoid combination with other NSAIDs
  • Minimize dose and duration of use
  • Risks in elderly people
  • GI bleeding, ulceration or perforation risk
  • Gastrointestinal infection risk
  • Impact on vitamin B12 absorption
  • Cardiovascular and cerebrovascular effects
  • Renal injury risk

Use in Special Populations

  • Increased unbound plasma fraction in elderly
  • Not recommended for moderate or severe renal impairment
  • Cautious use in mild to moderate hepatic impairment

Overdose Effects
