Medicine Details

Indications

  • Relief of signs & symptoms of osteoarthritis
  • Relief of signs & symptoms of rheumatoid arthritis
  • Relief of signs & symptoms of ankylosing spondylitis
  • Decrease the risk of developing gastric ulcers in patients at risk of developing NSAID-associated gastric ulcers

Pharmacology

  • Immediate release Esomeprazole Magnesium layer
  • Enteric-coated Naproxen core
  • Mechanism of action of Naproxen inhibits the prostaglandin synthesis
  • Proton pump inhibitor Esomeprazole suppresses gastric acid secretion

Dosage

  • Use the lowest effective dose for the shortest duration
  • Adult dosage: One tablet twice daily of either 375 mg naproxen/20 mg of esomeprazole or 500 mg naproxen/20 mg of esomeprazole
  • Adolescent dosage: 375 mg naproxen/20 mg of esomeprazole or 500 mg naproxen/20 mg of esomeprazole
  • Juvenile dosage: 375 mg naproxen/20 mg of esomeprazole

Administration

  • Take the tablet at least 30 minutes before meals
  • Do not split, chew, crush or dissolve the tablet

Interaction

  • Concomitant use of NSAIDs may reduce the antihypertensive effect of ACE inhibitors, diuretics & beta-blockers
  • Concomitant use of this tablet and warfarin may result in an increased risk of a bleeding complication
  • Esomeprazole may interfere with the absorption of drugs where gastric pH is an important determinant of bioavailability

Contraindications

  • Known hypersensitivity to any component of this tablet or substituted benzimidazoles
  • History of asthmay urticaria or other allergic-type reactions after taking aspirin or other NSAIDs
  • Use during the peri-operative period in the setting of coronary artery bypass graft (CABG) surgery

Side Effects

  • Decrease the occurrence of gastric ulcers and NSAID associated upper gastrointestinal adverse events compared to Naproxen alone
  • Clinical trial and epidemiological data suggest that use of coxibs and some NSAIDs may be associated with a small increased risk of arterial thrombotic events
  • Gastrointestinal side effects: nausea, vomiting, diarrhoea, flatulence, constipation, dyspepsia
  • Peptic ulcers, perforation or GI bleeding, particularly in older people, may occur

Pregnancy & Lactation

  • Naproxen and Esomeprazole tablet should not be given during the first and second trimester of pregnancy unless the potential benefit to the patient outweighs the potential risk to the foetus
  • Naproxen and Esomeprazole tablet is contraindicated during the third trimester of pregnancy
  • Naproxen and Esomeprazole tablet should not be used during breastfeeding

Precautions & Warnings

  • Avoid combination of Naproxen and Esomeprazole tablet and NSAIDs including cyclooxygenase-2 selective inhibitors
  • Undesirable effects may be minimized by using the lowest effective dose for the shortest duration necessary to control symptoms
  • Consideration for patients requiring concomitant low dose acetylsalicylic acid
  • Caution advised in patients receiving NSAIDs with concomitant medications which could increase the risk of ulceration or bleeding

Use in Special Populations

  • Use caution in elderly patients
  • Naproxen-containing products are not recommended for use in patients with moderate to severe or severe renal impairment
  • Monitor patients with mild to moderate hepatic impairment closely

Overdose Effects

  • Symptoms of Naproxen overdose: lethargy, dizziness, drowsiness, epigastric pain, abdominal discomfort
  • Symptoms of esomeprazole overdose are transient
  • Management for Naproxen overdose: symptomatic and supportive care
  • No specific antidote for Esomeprazole overdose
