Indications

  • Relief of signs and symptoms of osteoarthritis
  • Relief of signs and symptoms of rheumatoid arthritis
  • Relief of signs and symptoms of ankylosing spondylitis
  • Decrease the risk of developing gastric ulcers in patients at risk of developing NSAID-associated gastric ulcers

Pharmacology

  • Immediate release Esomeprazole Magnesium layer
  • Enteric-coated Naproxen core
  • Inhibition of prostaglandin synthesis
  • Proton pump inhibitor that suppresses gastric acid secretion

Dosage

  • Use the lowest effective dose
  • One tablet twice daily for adults
  • Different dosage for juvenile idiopathic arthritis in adolescent patients based on weight

Administration

  • Tablet should be taken at least 30 minutes before meals

Interactions

  • Reduced antihypertensive effect of ACE inhibitors, diuretics & beta-blockers with concomitant use of NSAIDs
  • Increased risk of bleeding complication with concomitant use of this tablet and warfarin
  • Interference with the absorption of drugs like Ketoconazole, iron salts, and digoxin by Esomeprazole

Contraindications

  • Known hypersensitivity to any component of this tablet or substituted benzimidazoles
  • History of asthmay urticaria or other allergic-type reactions after taking aspirin or other NSAIDs
  • Use during the peri-operative period in the setting of coronary artery bypass graft (CABG) surgery

Side Effects

  • Decreased occurrence of gastric ulcers and NSAID-associated upper gastrointestinal adverse events compared to Naproxen alone
  • Gastrointestinal side effects
  • Increased risk of arterial thrombotic events
  • Oedema, hypertension and cardiac failure
  • Peptic ulcers, perforation or GI bleeding

Pregnancy & Lactation

  • Contraindicated during the third trimester of pregnancy
  • Not recommended during breastfeeding
  • May impair female fertility

Precautions & Warnings

  • Avoid the combination of Naproxen and NSAIDs including cyclooxygenase-2 selective inhibitors
  • Use with low dose acetylsalicylic acid
  • Risk factors to develop NSAID related gastro-intestinal complications
  • Increased frequency of adverse reactions in older people

Use in Special Populations

  • Elderly patients
  • Patients with Moderate to Severe Renal impairment
  • Hepatic insufficiency
