Indications
- Relief of signs & symptoms of Osteoarthritis
- Relief of signs & symptoms of Rheumatoid arthritis
- Relief of signs & symptoms of Ankylosing spondylitis
- Decrease the risk of developing gastric ulcers in patients at risk of developing NSAID-associated gastric ulcers
Pharmacology
- Immediate release of Esomeprazole Magnesium layer
- Enteric-coated Naproxen core
- Inhibition of prostaglandin synthesis by Naproxen
- Suppression of gastric acid secretion by Esomeprazole
Dosage
- Adults: One tablet twice daily of either: 375 mg naproxen/20 mg of esomeprazole or 500 mg naproxen/20 mg of esomeprazole
- Adolescent Patients 12 Years of Age & Older: 375 mg naproxen/20 mg of esomeprazole or 500 mg naproxen/20 mg of esomeprazole if weight greater than 50 kg
- Adolescent Patients 12 Years of Age & Older: One tablet twice daily of 375 mg naproxen/20 mg of esomeprazole if weight 38 kg to less than 50 kg
Administration
- Tablet is to be taken at least 30 minutes before meals
Interaction
- Concomitant use of NSAIDs may reduce the antihypertensive effect of ACE inhibitors, diuretics & beta-blockers
- Concomitant use of this tablet and warfarin may result in an increased risk of a bleeding complication
- Inhibition of gastric acid secretion by Esomeprazole may interfere with the absorption of drugs where gastric pH is an important determinant of bioavailability
Contraindications
- Known hypersensitivity to any component of this tablet or substituted benzimidazoles
- History of asthmay urticaria or other allergic-type reactions after taking aspirin or other NSAIDs
- Use during the peri-operative period in the setting of coronary artery bypass graft (CABG) surgery
Side Effects
- Decreased occurrence of gastric ulcers and NSAID associated upper gastrointestinal adverse events compared to Naproxen alone
- Increased risk of arterial thrombotic events with use of coxibs and some NSAIDs
- Commonly observed gastrointestinal adverse events
- Oedema, hypertension and cardiac failure in association with NSAID treatment
- Gastrointestinal bleeding, ulceration or perforation, particularly in older people, may occur
Pregnancy & Lactation
- Contradicted during the third trimester of pregnancy
- Should not be used during breastfeeding
- Use of NSAIDs may impair female fertility
Precautions & Warnings
- Avoid combination with NSAIDs including cyclooxygenase-2 selective inhibitors
- Minimize undesirable effects by using the lowest effective dose for the shortest duration necessary to control symptoms
- Increased frequency of adverse reactions especially gastrointestinal bleeding, and perforation in older people
- Consider combination therapy with protective agents for patients with history of GI toxicity
- Caution advised in patients receiving NSAIDs with concomitant medications which could increase the risk of ulceration or bleeding
- Appropriate monitoring and advice required for patients with a history of hypertension and/or mild to moderate congestive heart failure
- Long-term administration of NSAIDs has resulted in renal papillary necrosis and other renal injury
Use in Special Populations
- Increased unbound plasma fraction of naproxen in the elderly
- Not recommended for use in patients with moderate to severe or severe renal impairment
- Use with caution and possible dose reduction based on hepatic impairment
Overdose Effects
- Symptoms of Naproxen overdose include lethargy, dizziness, drowsiness, epigastric pain, abdominal discomfort, heartburn, etc.
- Symptoms of Esomeprazole overdose are transient
- Management involves symptomatic and supportive care, there are no specific antidotes
