Indications

  • Osteoarthritis
  • Rheumatoid arthritis
  • Ankylosing spondylitis
  • Decrease the risk of developing gastric ulcers in patients at risk of developing NSAID-associated gastric ulcers

Pharmacology

  • Immediate release Esomeprazole Magnesium layer
  • Enteric-coated Naproxen core
  • NSAID with analgesic & antipyretic properties
  • Inhibition of prostaglandin synthesis
  • Proton pump inhibitor that suppresses gastric acid secretion

Dosage

  • Use the lowest effective dose
  • Adult dosage: 375 mg naproxen/20 mg of esomeprazole or 500 mg naproxen/20 mg of esomeprazole twice daily
  • Adolescent dosage: 375 mg naproxen/20 mg of esomeprazole or 500 mg naproxen/20 mg of esomeprazole (weight dependent)

Administration

  • Do not split, chew, crush or dissolve the tablet
  • Take at least 30 minutes before meals

Interaction

  • Reduced antihypertensive effect of ACE inhibitors, diuretics & beta-blockers with NSAIDs
  • Increased risk of bleeding complication with warfarin
  • Interference with the absorption of certain drugs like ketoconazole, iron salts, and digoxin
  • Effect of high-fat food on absorption

Contraindications

  • Known hypersensitivity to any component
  • History of allergic-type reactions after taking aspirin or other NSAIDs
  • Use during the peri-operative period in the setting of coronary artery bypass graft (CABG) surgery

Side Effects

  • Decrease in occurrence of gastric ulcers and NSAID associated upper gastrointestinal adverse events
  • Increased risk of arterial thrombotic events at high doses and in long-term treatment
  • Gastrointestinal adverse events such as peptic ulcers, perforation, GI bleeding, nausea, vomiting, diarrhoea, dyspepsia, abdominal pain
  • Cardiovascular, cerebrovascular, and renal effects

Pregnancy & Lactation

  • Contraindicated during the third trimester of pregnancy
  • Not recommended during breastfeeding
  • Potential impact on fertility

Precautions & Warnings

  • Avoid use with NSAIDs including cyclooxygenase-2 selective inhibitors
  • Minimize undesirable effects with lowest effective dose
  • Special considerations for older people, gastrointestinal effects, cardiovascular and cerebrovascular effects, renal effects

Use in Special Populations

  • Potential dosage adjustment needed for elderly patients
  • Not recommended for use in patients with moderate to severe or severe renal impairment
  • Close monitoring for patients with hepatic insufficiency
