Indications

  • Relief of signs and symptoms of Osteoarthritis
  • Relief of signs and symptoms of Rheumatoid arthritis
  • Relief of signs and symptoms of Ankylosing spondylitis
  • Decrease the risk of developing gastric ulcers in patients at risk of developing NSAID-associated gastric ulcers

Pharmacology

  • Immediate release Esomeprazole Magnesium layer
  • Enteric-coated Naproxen core
  • Inhibition of prostaglandin synthesis
  • Reduction of gastric acidity

Dosage

  • Use of lowest effective dose
  • Adult dosage: 375 mg naproxen/20 mg esomeprazole twice daily
  • Adolescent dosage: 375 mg naproxen/20 mg esomeprazole for weight greater than 50 kg

Administration

  • Take at least 30 minutes before meals

Interaction

  • Reduced antihypertensive effect of ACE inhibitors, diuretics, and beta-blockers with concomitant use of NSAIDs
  • Increased risk of bleeding complication with concomitant use of warfarin
  • Interference with drug absorption by Esomeprazole

Contraindications

  • Hypersensitivity to any component of the tablet
  • History of allergic-type reactions after taking aspirin or other NSAIDs
  • Use during peri-operative period in the setting of coronary artery bypass graft (CABG) surgery

Side Effects

  • Decreased occurrence of gastric ulcers and NSAID associated upper gastrointestinal adverse events
  • Possible association with arterial thrombotic events
  • Gastrointestinal adverse events
  • Oedema, hypertension, and cardiac failure

Pregnancy & Lactation

  • Avoidance during first and second trimester of pregnancy
  • Contraindicated during the third trimester of pregnancy
  • Avoidance during breastfeeding
  • Potential impairment of female fertility

Precautions & Warnings

  • Avoid concomitant use of NSAIDs and cyclooxygenase-2 selective inhibitors
  • Use with low dose acetylsalicylic acid
  • Consideration of protective agents for high-risk patients
  • Regular surveillance for long-term treatment patients

Use in Special Populations

  • Caution with high doses in elderly patients
  • Not recommended for patients with moderate to severe renal impairment
  • Close monitoring in patients with mild to moderate hepatic impairment

Overdose Effects

  • Symptomatic and supportive care for NSAID overdose
