Indications

  • Relief of signs and symptoms of osteoarthritis
  • Relief of signs and symptoms of rheumatoid arthritis
  • Relief of signs and symptoms of ankylosing spondylitis
  • Decrease the risk of developing gastric ulcers in patients at risk of developing NSAID-associated gastric ulcers

Pharmacology

  • Immediate release Esomeprazole Magnesium layer
  • Enteric-coated Naproxen core
  • Inhibition of prostaglandin synthesis
  • Suppression of gastric acid secretion
  • Specific inhibition of the H+/k+ -ATPase in the gastric parietal cell
  • Reduction of gastric acidity

Dosage

  • Use the lowest effective dose for the shortest duration
  • Different dosage for adults and adolescent patients
  • Dosage based on weight
  • Tablet to be taken at least 30 minutes before meals

Administration

  • Do not split, chew, crush or dissolve the tablet

Interaction

  • Reduction of antihypertensive effect of ACE inhibitors, diuretics, and beta-blockers
  • Increased risk of bleeding complication with warfarin
  • Interference with absorption of drugs like Ketoconazole, iron salts, and digoxin
  • Effect of administration with high-fat food on absorption of naproxen

Contraindications

  • Known hypersensitivity to any component of the tablet
  • History of asthmay urticaria or other allergic-type reactions after taking aspirin or other NSAIDs
  • Use during the peri-operative period in the setting of coronary artery bypass graft (CABG) surgery

Side Effects

  • Decrease the occurrence of gastric ulcers and NSAID associated upper gastrointestinal adverse events
  • Possible association with arterial thrombotic events
  • Reported adverse events include gastrointestinal issues, oedema, hypertension, and cardiac failure

Pregnancy & Lactation

  • Not recommended during the third trimester of pregnancy
  • Unknown excretion of esomeprazole in human milk
  • Potential impairment of female fertility

Precautions & Warnings

  • Avoidance of combination with other NSAIDs
  • Minimization of undesirable effects by using the lowest effective dose for the shortest duration
  • Regular surveillance for long-term treatment patients
  • Caution in older people due to increased frequency of adverse reactions

Use in Special Populations

  • Possible dosage adjustment in elderly patients
  • Not recommended for patients with moderate to severe renal impairment
  • Careful monitoring in patients with mild to moderate hepatic impairment

Overdose Effects

  • Symptoms such as lethargy, dizziness, drowsiness, and gastrointestinal issues
  • Absence of specific antidotes for naproxen overdose
  • Limited experience of esomeprazole overdose
