Indications
- Relief of Osteoarthritis symptoms
- Relief of Rheumatoid arthritis symptoms
- Relief of Ankylosing spondylitis symptoms
- Decrease the risk of developing gastric ulcers in patients at risk of developing NSAID-associated gastric ulcers
Pharmacology
- Immediate release Esomeprazole Magnesium layer
- Enteric-coated Naproxen core
- NSAID with analgesic & antipyretic properties
- Inhibits prostaglandin synthesis
- Proton pump inhibitor that suppresses gastric acid secretion
Dosage
- Consider potential benefits & risks
- Use lowest effective dose for shortest duration
- Adult dosage: 375 mg naproxen/20 mg esomeprazole or 500 mg naproxen/20 mg esomeprazole twice daily
- Adolescent dosage: 375 mg naproxen/20 mg esomeprazole or 500 mg naproxen/20 mg esomeprazole based on weight
Administration
- Take at least 30 minutes before meals
- Do not split, chew, crush or dissolve the tablet
Interaction
- May reduce antihypertensive effect of ACE inhibitors, diuretics & beta-blockers
- May increase risk of bleeding complication with warfarin
- May interfere with absorption of drugs where gastric pH is important
- Ingestion with high-fat food prolongs absorption & decreases peak plasma concentration
Contraindications
- Known hypersensitivity to any component of the tablet or substituted benzimidazoles
- History of asthmay urticaria or allergic-type reactions after taking aspirin or other NSAIDs
- Use during the peri-operative period in the setting of coronary artery bypass graft (CABG) surgery
Side Effects
- Decreased incidence of gastrointestinal side effects
- Lower risk of arterial thrombotic events compared to Naproxen alone
- Possible adverse events: Oedema, hypertension, cardiac failure, gastrointestinal issues
Pregnancy & Lactation
- Avoid during first and second trimester of pregnancy
- Contraindicated during third trimester of pregnancy
- Not recommended during breastfeeding
- May impair female fertility
Precautions & Warnings
- Avoid in combination with other NSAIDs
- Minimize undesirable effects with lowest effective dose
- Regular surveillance for long-term treatment patients
- Increased frequency of adverse reactions in older people
- Caution in patients with history of gastrointestinal disease, hypertension, and renal impairment
Use in Special Populations
- Use caution and possible dose adjustment in elderly patients
- Not recommended in patients with moderate to severe renal impairment
- Monitor patients with mild to moderate hepatic impairment closely
Overdose Effects
- Symptoms may include lethargy, dizziness, epigastric pain, and gastrointestinal bleeding
- Management: Symptomatic and supportive care, with no specific antidotes
