Indications

  • Decrease the risk of developing gastric ulcers

Mode of Action

  • Immediate release Esomeprazole Magnesium layer
  • Enteric-coated Naproxen core

Dosage

  • Carefully consider potential benefits & risks
  • Use lowest effective dose
  • Different treatment for lower dose of Esomeprazole
  • Dosage for Rheumatoid Arthritis, Osteoarthritis, and Ankylosing Spondylitis
  • Dosage for Juvenile Idiopathic Arthritis in Adolescent Patients based on weight

Administration

  • Do not split, chew, crush, or dissolve the tablet
  • Take at least 30 minutes before meals

Interaction

  • Concomitant use of NSAIDs may reduce the antihypertensive effect of ACE inhibitors, diuretics & beta-blockers
  • Concomitant use with warfarin may increase risk of bleeding
  • Inhibition of gastric acid secretion & interference with drug absorption by Esomeprazole
  • Effects of administration with high-fat food in healthy volunteers

Contraindications

  • Known hypersensitivity to any component of the tablet or substituted benzimidazoles
  • History of asthmay urticaria or other allergic-type reactions after taking aspirin or other NSAIDs
  • Use during the peri-operative period in the setting of coronary artery bypass graft (CABG) surgery

Side Effects

  • Gastrointestinal side effects
  • Decreased occurrence of gastric ulcers and NSAID associated upper gastrointestinal adverse events compared to Naproxen alone
  • Risk of arterial thrombotic events
  • Adverse cardiovascular and cerebrovascular effects
  • Renal effects
  • Oedema, hypertension, and cardiac failure
  • Gastrointestinal bleeding, ulceration, or perforation
  • Nausea, vomiting, and diarrhea

Pregnancy & Lactation

  • Not recommended during the first and second trimester of pregnancy
  • Contraindicated during the third trimester of pregnancy
  • Not recommended during breastfeeding
  • NSAIDs like Naproxen may impair female fertility

Precautions & Warnings

  • Avoid combination with NSAIDs and cyclooxygenase-2 selective inhibitors
  • Use with low dose acetylsalicylic acid
  • General caution in older people
  • Consideration in patients with gastrointestinal disease
  • Cardiovascular and cerebrovascular monitoring for patients with hypertension and congestive heart failure
  • Monitoring for patients with renal impairment
  • Possible dose reduction based on hepatic insufficiency

Use in Special Populations

  • Use caution and possible dosage adjustment in elderly patients
  • Not recommended in patients with moderate to severe renal impairment
  • Close monitoring and possible dose reduction in patients with mild to moderate hepatic impairment

Overdose Effects

  • Symptoms related to Naproxen overdose
  • Symptoms related to Esomeprazole overdose
  • Management of overdose including symptomatic and supportive care, emesis, or activated charcoal
