Indications
- Relief of signs & symptoms of Osteoarthritis
- Relief of signs & symptoms of Rheumatoid arthritis
- Relief of signs & symptoms of Ankylosing spondylitis
- Decrease the risk of developing gastric ulcers in patients at risk of developing NSAID-associated gastric ulcers
Pharmacology
- Consists of immediate release Esomeprazole Magnesium layer & enteric-coated Naproxen core
- Naproxen: NSAID with analgesic & antipyretic properties
- Esomeprazole: Proton pump inhibitor that suppresses gastric acid secretion
Dosage
- Use lowest effective dose for shortest duration consistent with individual patient treatment goals
- Adult dose: One tablet twice daily of either 375 mg naproxen/20 mg of esomeprazole or 500 mg naproxen/20 mg of esomeprazole
- Adolescent patients 12 years of age & older dose: Based on weight, 375 mg naproxen/20 mg of esomeprazole or 500 mg naproxen/20 mg of esomeprazole
Administration
- Tablet is to be taken at least 30 minutes before meals
Interaction
- Concomitant use of NSAIDs may reduce antihypertensive effect of ACE inhibitors, diuretics & beta-blockers
- Concomitant use with warfarin may result in increased risk of bleeding
- Inhibition of gastric acid secretion & interference with absorption of certain drugs
- Administration with high-fat food affects absorption of Naproxen
Contraindications
- Known hypersensitivity to any component of the tablet or substituted benzimidazoles
- History of asthmay urticaria or other allergic-type reactions after taking aspirin or other NSAIDs
- Use during the peri-operative period in the setting of coronary artery bypass graft (CABG) surgery
Side Effects
- Decreased occurrence of gastric ulcers and NSAID associated upper gastrointestinal adverse events
- Possible increased risk of arterial thrombotic events with long-term use
- Commonly observed adverse events are gastrointestinal in nature, including peptic ulcers, perforation, GI bleeding, nausea, vomiting, diarrhea, etc.
Pregnancy & Lactation
- Not recommended during first, second, and third trimester of pregnancy
- Not recommended for use during breastfeeding
- Use may impair female fertility
Precautions & Warnings
- Avoid use with other NSAIDs and cyclooxygenase-2 selective inhibitors
- Minimize undesirable effects by using lowest effective dose for shortest duration necessary
- Consider risk factors for NSAID-related gastro-intestinal complications
- Increased adverse reactions in older people
- Consideration for combination therapy with protective agents for high-risk patients
- Special caution for patients with particular medical conditions
Use in Special Populations
- Caution and possible dose adjustment in elderly patients
- Not recommended for use in patients with moderate to severe renal impairment
- Monitor and consider dose reduction in patients with hepatic insufficiency

- Symptoms and management related to Naproxen and Esomeprazole overdose
- No known specific antidotes for either component
