Indications

  • Relief of signs & symptoms of osteoarthritis
  • Relief of signs & symptoms of rheumatoid arthritis
  • Relief of signs & symptoms of ankylosing spondylitis
  • Decrease the risk of developing gastric ulcers in patients at risk of developing NSAID-associated gastric ulcers

Pharmacology

  • Consists of immediate release Esomeprazole Magnesium layer & enteric-coated Naproxen core
  • Naproxen is a NSAID with analgesic & antipyretic properties
  • Esomeprazole is a proton pump inhibitor that suppresses gastric acid secretion by specific inhibition of the H+/k+ -ATPase in the gastric parietal cell

Dosage

  • Use the lowest effective dose for the shortest duration consistent with individual patient treatment goals
  • Adult dosage: 375 mg naproxen/20 mg of esomeprazole twice daily or 500 mg naproxen/20 mg of esomeprazole twice daily
  • Adolescent patients dosage: 375 mg naproxen/20 mg of esomeprazole or 500 mg naproxen/20 mg of esomeprazole depending on weight

Administration

  • Do not split, chew, crush or dissolve the tablet
  • Take at least 30 minutes before meals

Interaction

  • Concomitant use of NSAIDs may reduce the antihypertensive effect of ACE inhibitors, diuretics & beta-blockers
  • Concomitant use with warfarin may result in increased risk of bleeding
  • Inhibition of gastric acid secretion may interfere with the absorption of drugs where gastric pH is an important determinant of bioavailability

Contraindications

  • Known hypersensitivity to any component of the tablet or substituted benzimidazoles
  • History of asthmay urticaria or other allergic-type reactions after taking aspirin or other NSAIDs
  • Use during the peri-operative period in the setting of coronary artery bypass graft (CABG) surgery

Side effects

  • Clinical trial and epidemiological data suggest that use of coxibs and some NSAIDs may be associated with a small increased risk of arterial thrombotic events
  • Gastrointestinal side effects include nausea, vomiting, diarrhea, flatulence, constipation, dyspepsia, abdominal pain, melaena, haematemesis, ulcerative stomatitis, exacerbation of colitis and Crohn’s disease

Pregnancy lactation

  • Tablet should not be given unless potential benefit outweighs potential risk to the fetus during pregnancy
  • Tablet should not be used during breastfeeding
  • Use of NSAIDs may impair female fertility

Precautions warnings

  • Avoid combination with other NSAIDs including cyclooxygenase-2 selective inhibitors due to cumulative risks of inducing serious NSAID-related adverse events
  • Undesirable effects may be minimized by using the lowest effective dose for the shortest duration necessary to control symptoms
  • Risk-factors for developing NSAID related gastro-intestinal complications include high age, concomitant use of anticoagulants, corticosteroids, and debilitating cardiovascular disease

Use in special populations

  • Elderly patients may require dosage adjustment
  • Not recommended in patients with moderate to severe or severe renal impairment
  • Monitor patients with mild to moderate hepatic impairment closely

Overdose effects

  • Symptoms of naproxen overdose include lethargy, dizziness, heartburn, nausea, and gastrointestinal bleeding
  • Symptoms of esomeprazole overdose are transient
  • Treatment should be symptomatic and general supportive measures should be utilized for overdose
