Pharmacology
- Non-steroidal anti-inflammatory drug (NSAID)
- Anti-inflammatory

Dosage
- Standard dosage of 20mg daily
- Acute gout treatment dose of 40mg followed by 20mg
- Progressive increase in response over time
- Avoiding daily doses higher than 20mg for chronic disorders
- Special dosage instructions for elderly, kidney or liver disease patients
Administration
- Tablets to be taken with water
- Preferable to take after a meal
Interaction
- Salicylates displacing the drug from protein-binding sites
- Concurrent treatment with other NSAIDs
- Caution with concurrent administration of methotrexate
- Potential interaction with lithium, K-sparing diuretics, furosemide
- Attenuation of antihypertensive effects of alpha-adrenergic blockers and ACE-inhibitors
Contraindications
- Hypersensitivity to the drug
- Induction of symptoms of asthma, rhinitis, or urticaria by salicylates or other NSAIDs
- Diseases of the upper gastrointestinal tract
Side Effects
- Gastrointestinal discomfort, dyspepsia, heartburn, nausea
- Dizziness, headache
- Constipation, diarrhea, stomatitis, vomiting, ulcers
- Skin itching, erythema, rash, urticaria
- Increase in BUN or creatinine, edema
- Elevated liver enzyme activity
- Possible gastrointestinal bleeding, visual disturbances, anemia, thrombocytopenia
- Hypersensitivity reactions including dyspnea, anaphylaxis, angioedema
Pregnancy & Lactation
- Inhibition of prostaglandin synthesis during late pregnancy
- Avoidance of treatment during the third trimester
- Very small amount passing into breast milk
- Need for weaning or discontinuation of drug
Precautions & Warnings
- Inhibition of renal prostaglandin synthesis impacting renal hemodynamics
- Inhibition of platelet aggregation potentially affecting hemostasis
- Monitoring patients with coagulation disorders or gastrointestinal disease
- Immediate withdrawal in case of peptic ulceration or gastrointestinal bleeding
- Ophthalmic evaluation for patients with visual disturbances
- Caution with decreased plasma albumin levels, detection of infection, and dislike/tolerance of milk products
Use in Special Populations
- Not recommended for use in patients under 16 years of age
- Impact on ability to drive and use machines
