Dosage
- Standard dosage of 20 mg daily
- Acute gout dosage of 40 mg for 2 days followed by 20 mg for 5 days
- Therapeutic effect evident early in treatment
- Progressive increase in response over time
- Avoid daily doses higher than 20 mg for chronic disorders
- Reduction to 10 mg for maintenance in long-term treatment
- Applicable to elderly, kidney or liver disease patients
Administration
Take with a glass of water, preferably during or immediately after a meal
Interaction
- Salicylate displacement
- Increased risk of gastrointestinal reactions
- Caution with methotrexate
- No significant interaction with gold, penicillamine, probenecid, furosemide, atenolol, digoxin, antacids, cimetidine, warfarin, phenprocoumon, oral antidiabetic drugs, low molecular weight heparin
Contraindications
- Hypersensitivity
- Salicylate or NSAID-induced symptoms
- Upper gastrointestinal disease
Side Effects
- Gastrointestinal discomfort, dyspepsia, nausea
- Dizziness, headache
- Constipation, diarrhea, vomiting
- Skin itching, rash, urticaria
- Increase in BUN or creatinine, edema
- Elevated liver enzyme activity
- Palpitations
- Rare cases of GI perforation, visual disturbances, anemia, asthma, anaphylaxis, elevated blood pressure, hepatitis
Pregnancy & Lactation
- Inhibitory effect on prostaglandin synthesis
- Avoid treatment in third trimester
- Small amount in breast milk
- No adverse reactions in breast-fed infants
Precautions & Warnings
- Inhibition of renal prostaglandin synthesis
- Monitoring of renal and cardiac function
- Caution with coagulation disorders
- Close monitoring for gastrointestinal disease
- Immediate withdrawal in case of ulceration or bleeding
- Discontinue treatment in case of severe skin reactions
- Ophthalmic evaluation for visual disturbances
- Caution with reduced plasma albumin levels
- Masking of signs of infection
- Avoid in patients who dislike or cannot tolerate milk products
Use in Special Populations
- Not recommended for use in patients under 16 years of age
- Effects on ability to drive and use machines
